心外膜脂肪厚度对急性冠状动脉综合征的诊断价值

【摘要】:目的 探讨心外膜脂肪厚度对急性冠状动脉综合征(ACS)的诊断价值。方法 选择120例ACS患者,其中单支病变、双支病变、多支病变各40例;另择心功能正常的健康人40例作为对照组。采用彩色多普勒超声诊断仪测定入选者心外膜脂肪厚度,采用受试者工作特征曲线(ROC)分析心外膜脂肪厚度对ACS的诊断价值。结果ACS单支病变、双支病变、多支病变患者的心外膜脂肪厚度均明显高于对照组(P均0.01)。单支、双支、多支病变患者的心外膜脂肪厚度呈递增趋势,三组间比较有统计学差异(P均0.05)。ROC分析显示,心外膜脂肪厚度对ACS单支病变、双支病变、多支病变诊断的AUC值分别为0.697、0.814、0.837。结论 超声心动图测定的心外膜脂肪厚度与ACS病变程度有关,对ACS双支病变和多支病变具有较好的诊断价值。
[Abstract]:Objective to evaluate the diagnostic value of epicardial fat thickness in acute coronary syndrome (ACS). Methods 120 patients with ACS were selected, including 40 patients with single vessel disease, 40 patients with double vessel disease and 40 patients with multiple vessel disease, and 40 healthy persons with normal cardiac function as control group. The epicardial fat thickness was measured by color Doppler ultrasound, and the diagnostic value of epicardial fat thickness to ACS was analyzed by using the operating characteristic curve (ROC). Results the thickness of epicardial fat in ACS patients with single vessel disease, double vessel disease and multivessel lesion was significantly higher than that in control group (P 0.01). The thickness of epicardial fat in patients with single vessel, double vessel and multiple vessel lesions showed an increasing trend, and there were significant differences among the three groups (all P 0. 05). ROC analysis showed that epicardial fat thickness was associated with single vessel lesion and double vessel lesion of ACS, P < 0. 05). The AUC values of multivessel lesions were 0. 697 卤0. 814 and 0. 837 respectively. Conclusion the thickness of epicardial fat measured by echocardiography is related to the degree of ACS lesions. It has a good value in the diagnosis of double vessel lesions and multivessel lesions of ACS.
