无创血管功能检测在早期动脉硬化患者中的应用及相关因素分析

【摘要】:目的:通过无创血管功能筛检技术在早期动脉硬化患病人群中的运用,探讨无创血管功能检测指标--脉搏波传导速度在衡量动脉硬化危害及动脉弹性异常程度中的应用价值,分析与其主要影响因素之间的相关性,以便为动脉硬化的早期防治提供一些肯定的参考依据。方法:收集于皖南医学院附属医院弋矶山医院健康体检中心在2015年1月-2016年7月期间进行体格检查者601例(男性358例,女性243例,年龄范围在21岁-80岁之间)作为研究对象。采用VBP-9 120806005动脉硬化检测仪(北京鑫悦琦科贸有限公司)检验臂(肱)踝脉搏波传导速度(brachial ankle pulse wave velocity,baPWV)和踝臂指数(ankle brachial index,ABI),同步记录反射波增强指数(augmentation index,AI)和心率(heart rate,HR),并使用该仪器内自带的调查卷同步采集被检者其它有关信息。根据臂踝脉搏波传导速度(brachial ankle pulse wave velocity,baPWV)1400cm/s为异常的基准值[1],以左右两侧较高测量值为定组标准,将受检者界定为动脉硬化组和非动脉硬化组,其中baPWV1400cm/s为动脉硬化组,共301例,baPWV≤1400cm/s为非动脉硬化组,共300例。采集被检者清晨进餐前静脉血进行尿酸、肌酐、肾小球滤过率、胱抑素C、血糖、糖化血红蛋白、血脂等生化标志物的检测,并记录结果。采用SPSS 18.0统计学分析软件进行t检验、卡方检验及Logistic回归分析等,以P0.05为差异有统计学意义。结果:1、动脉硬化组和非动脉硬化组临床资料对比得出:baPWV与年龄、体重、BMI、SBP、DBP、MAP、HR、肌酐、尿酸、胱抑素C、BS、HBA1C、TC、TG及LDL-C存在正性关联,与L-ABI、R-ABI、HDL-C及GFR呈负性关联。2、baPWV与各因素的Logistic回归分析显示:年龄、SBP、血尿酸、胱抑素C和血糖是baPWV的独立影响因素。结论:1.年龄、SBP、血尿酸、血糖和胱抑素C是baPWV的独立影响因素。2.年龄及收缩压与动脉硬化的严重度呈正相关,即随着年龄及收缩压的增加动脉弹性异常度越重。3.血尿酸及胱抑素C浓度与动脉硬化之间存在正性关联,其浓度高低能反应动脉硬化病变的严重度。

The age range ranged from 21 to 80 years of age.
