微量白蛋白尿与心血管疾病

【摘要】:微量白蛋白尿(microalbuminuria,MAU)的出现不仅提示肾脏血管内皮受损,还可作为全身血管内皮受损的信号,是血管内皮功能障碍及血管炎症反应的早期预测因子。微量白蛋白尿在心血管疾病中,与心血管危险因素及心血管疾病关系密切,对动脉粥样硬化、冠心病、高血压、心力衰竭的病情评估及预后有重要的价值。但其是如何与心血管疾病有关的,目前尚未阐明,仍需要深入的基础研究。对于如何减少蛋白尿方面,目前在实际医疗工作中应用药物主要有四种:血管紧张素转换酶抑制剂、血管紧张素II受体拮抗剂、肾素抑制剂、醛固酮受体拮抗剂。但是否可通过减低蛋白尿来明显延缓心血管疾病的进展和改善患者预后,为心血管疾病的预防与治疗带来新的思路,仍需要更多的大数据样本及高质量的临床试验来证实。
[Abstract]:The appearance of microalbuminuria (microalbuminuria,MAU) not only suggests that renal vascular endothelial damage, but also can be used as a signal of systemic vascular endothelial damage, and is an early predictor of vascular endothelial dysfunction and vascular inflammation. Microalbuminuria is closely related to cardiovascular risk factors and cardiovascular diseases in cardiovascular diseases. It is of great value in the evaluation and prognosis of atherosclerosis, coronary heart disease, hypertension and heart failure. However, how it is related to cardiovascular disease has not yet been clarified, and further basic research is still needed. At present, there are four kinds of drugs used in practical medical work: angiotensin converting enzyme inhibitor, angiotensin II receptor antagonist, renin inhibitor and aldosterone receptor antagonist. However, whether we can significantly delay the progress of cardiovascular disease and improve the prognosis of patients by reducing albuminuria, and bring new ideas for the prevention and treatment of cardiovascular disease, more big data samples and high quality clinical trials are still needed to confirm it.
