人工耳蜗植入及其相关因素对残余听力的影响

【摘要】: 目的:初步探讨人工耳蜗植入对患者不同频率残余听力的影响,并分析人工耳蜗植入术影响残余听力的可能因素。 方法:分析9例资料完整,术前纯音测听存在残余听力,中耳、内耳无明显异常、术后无并发症的单侧人工耳蜗植入者的残余听力的变化。 结果:植入侧手术后的残余听力下降,在不同频率之间有显著性差异,于250Hz、500Hz、1KHz处较为显著,分别丧失15,17,8dB HL;植入侧及非植入侧于4 KHz、8KHz两个频率的手术前后纯音听阈均有显著性差异;直电极与弯电极两组于各频率的残余听力损失均无显著性差异。 结论:人工耳蜗植入术对不同的频率会造成不同程度的损害,且1KHz以下频率受损较明显。此现象可能与电极植入过程中损害内耳结构和功能,以及术后耳蜗内广泛炎症等有一定关系。
[Abstract]:Objective: to explore the effect of cochlear implantation on residual hearing in patients with different frequencies and to analyze the possible factors affecting residual hearing. Methods: the data of 9 cases of unilateral cochlear implants without complications were analyzed. The residual hearing was not abnormal in the middle ear and the inner ear in pure tone audiometry before operation and there was no complication after operation. Results: there was a significant difference in residual hearing loss between different frequencies after implantation operation, and it was more significant at 250 Hz ~ 500 Hz ~ 1 kHz. There were significant differences in pure tone hearing threshold between the implanted side and the non-implanted side at 4 KHz and 8 kHz, respectively, and there was no significant difference between the straight electrode group and the curved electrode group in the residual hearing loss at each frequency. Conclusion: cochlear implantation can cause different degrees of damage to different frequencies, and the frequency below 1kHz is more obvious. This phenomenon may be related to the damage to the structure and function of inner ear during electrode implantation. And postoperative cochlea extensive inflammation and so on has the certain relation.
