屈光参差性弱视患者静息态fMRI功能连接研究

【摘要】: 目的: 运用功能磁共振成像(functional magnetic resonance imaging, fMRI)技术,采用种子点功能连接方法分析屈光参差性弱视患者和正常人在静息状态下不同脑区间相关性的差异。 材料与方法: 选择左眼屈光参差性弱视患者13例,正常对照组17例。采用GE 1.5T磁共振扫描仪对所有被试进行静息态fMRI研究。应用AFNI、Matlab、SPM软件进行预处理和统计分析。预处理包括时间校正、头动校正、空间标准化、空间平滑、去线性漂移、低通滤波和去除协变量。选择左侧BA19区(-30、-75、42)作为功能连接种子点进行分析,对其与全脑其它脑区所有体素的时间序列做相关分析,产生一个空间的互相关图(功能连接模式图)。采用单样本t-检验和双样本t-检验进行组内和组间分析。统计参数图叠加到MNI标准三维模板脑进行显示,用已知的神经解剖学标记来描述与左侧BA19(-30、-75、42)区有显著相关的区域。观察有统计学意义的团块(mass)的大小、所在脑区及其坐标(MNI坐标)。记录相关区的体积(cluster)及相关强度(用t检验统计值“T”表示,T值越大,相关性越强)。 结果: 在严格控制被试头动、行为学表现及机械噪声等因素影响后,最终符合实验要求的被试为左眼屈光参差性弱视患者12例,正常人16例。为达到组间性别及年龄的匹配,选择左眼屈光参差性弱视患者组和正常对照组内各12例进行分析,以保证实验结果的可靠性。 正常对照组与左侧BA19区正相关的脑区主要为:右侧小脑半球、双侧梭状回、双侧颞中回、双侧颞下回、双侧额上回、双侧楔前叶、双侧顶上小叶、双侧顶下小叶、双侧扣带回后部、双侧额中回;左眼屈光参差性弱视患者组与左侧BA19区正相关的脑区有:右侧小脑半球、左侧梭状回、左侧额下回、双侧额中回、双侧楔前叶、双侧顶上小叶、双侧顶下小叶;正照对照组与左侧BA19区的正相关性高于患者组的脑区主要包括:左侧小脑半球、右侧尾状核、左侧额中回、左侧顶上小叶、左侧顶下小叶、左侧额上回。 结论: 1.正常人在静息状态下存在与左侧BA19区具有相关性的神经网络; 2.左眼屈光参差性弱视患者左侧小脑半球、右侧尾状核、左侧额中回、左侧顶上小叶、左侧顶下小叶、左侧额上回与左侧BA19区功能连接程度减低,可能是左眼屈光参差性弱视患者的发生机制之一。
[Abstract]:Objective:. Functional magnetic resonance imaging (fMRI) technique was used to analyze the correlation between anisometropia amblyopia and normal subjects in different brain regions. Materials and methods:. Thirteen cases of anisometropia amblyopia in left eye and 17 cases of normal control group were selected. All subjects were studied by GE 1.5T magnetic resonance scanner in resting state fMRI. The pretreatment and statistical analysis were performed with AFNINT-Matlab-SPM software. The pretreatment included time correction. Head-moving correction, spatial standardization, spatial smoothing, de-linear drift, low-pass filtering and removal of covariates. Select the left BA19 region -30- 75N42as the functional connection seed point for analysis, Correlation analysis was made between the time series and all voxels in other brain regions of the whole brain. Generating a spatial cross-correlation map (functional connection pattern map). Using single-sample t-test and double-sample t-test for intra-group and inter-group analysis. The statistical parameter map is superimposed on the MNI standard three-dimensional template brain for display. The known neuroanatomical markers were used to describe the areas significantly related to the left side of the mass mass, BA19 ~ (-30) ~ (-75) ~ (42). The size of a statistically significant mass was observed. In the brain area and its coordinate, MNI coordinate, the volume of correlation area is recorded and the correlation strength is recorded. The statistical value "T" of t test indicates that the greater the value of T is, the stronger the correlation is. Results:. After strictly controlling the effects of the factors such as head movement, behavioral performance and mechanical noise, the subjects who met the experimental requirements were 12 patients with anisometropic amblyopia of left eye and 16 normal subjects. 12 cases of anisometropia amblyopia in left eye and 12 cases of normal control group were analyzed to ensure the reliability of the experimental results. In the control group, the positive correlation between the left BA19 and the right cerebellar hemisphere, bilateral fusiform gyrus, bilateral middle temporal gyrus, bilateral infratemporal gyrus, bilateral superior frontal gyrus, bilateral anterior cuneate lobe, bilateral superior parietal lobule, bilateral inferior parietal lobule were observed. The right cerebellar hemisphere, left fusiform gyrus, left inferior frontal gyrus, bilateral middle frontal gyrus, bilateral anterior wedge lobe, bilateral superior parietal lobule, left cerebellar hemisphere, left fusiform gyrus, bilateral frontal gyrus, bilateral anterior wedge lobe, bilateral parietal lobule, The positive correlation between the positive control group and the left BA19 area was higher than that in the patient group, including left cerebellar hemisphere, right caudate nucleus, left middle frontal gyrus, left superior parietal lobule, left inferior parietal lobule, left superior frontal gyrus, and left superior frontal gyrus. Conclusion:. 1. There is a neural network with correlation between the left BA19 region and the rest state. 2. Left cerebellar hemisphere, right caudate nucleus, left middle frontal gyrus, left superior parietal lobule, left inferior parietal lobule, left superior frontal gyrus, left superior frontal gyrus, left superior frontal gyrus and left BA19 area were decreased in patients with left anisometropic amblyopia. It may be one of the mechanisms of anisometropia amblyopia in the left eye.
