比较飞秒激光辅助角膜移植术与传统角膜移植术的Meta分析

【摘要】:目的本文旨在用meta分析观察飞秒激光辅助角膜移植手术的疗效及其安全性,并与传统角膜移植手术进行比较。 方法采用Cochrane系统评价方法,计算机检索PubMed, Embase,Cochrane Central和OVID等英文数据库,符合入选标准的文献由2名评价者独立依据“Cochrane偏倚风险评估工具”评价RCT研究,依据“MINORS”标准评价非RCT研究,并交叉核对纳入研究的质量,采用RevMan5.2.6软件进行meta分析。 结果7篇文献被纳入meta分析,结果显示飞秒激光辅助角膜移植术与传统角膜移植术的术后短期散光值(MD=-1.59,95%CI [-2.86,-0.31], P=0.01)和长期散光值(MD=-1.17,95%CI [-1.97,-0.38], P=0.004)的差异有统计学意义,但飞秒激光在术后短期最佳矫正视力(MD=0.01,95%CI [-46,0.48], P=0.96)、长期最佳矫正视力(MD=0.10,95%CI [-0.06,0.26], P=0.23)、排斥率(RR=0.57,95%CI [0.13,2.49], P=0.46)及移植失败率(RR=1.74,95%CI [0.74,4.07], P=0.2)方面与传统角膜移植术相比差异无统计学意义。 结论与传统角膜移植术相比,,飞秒激光辅助角膜移植术能明显降低术后散光,但不能提高术后最佳矫正视力。两种手术方式的排斥率和移植失败率均较低且没有明显差异。
[Abstract]:Objective to observe the efficacy and safety of femtosecond laser assisted keratoplasty by meta and compare it with traditional keratoplasty. Methods Cochrane system was used to search the English database of PubMed, Embase Cochrane Central and OVID. According to the selection criteria, the RCT study was evaluated by two evaluators independently according to the Cochrane bias risk Assessment tool, and the non-#en6# study was evaluated according to the MINORS criteria. And cross-check into the quality of the study, using RevMan5.2.6 software for meta analysis. Results Seven articles were included in the meta analysis. The results showed that there were significant differences in short-term astigmatism between femtosecond laser assisted keratoplasty and conventional keratoplasty, such as MD-1.59 ~ 95CI [-2.86 ~ 0.31, P ~ (0.01)] and long-term astigmatism: MD-1.17 ~ 95CI [-1.97 -0.38], P _ (0.004). However, femtosecond laser has no significant difference compared with traditional keratoplasty in terms of short-term best corrected visual acuity (BCVA): MD0.01 + 95CI [-46 卤0.48], P0. 96, MD0.1095 + CI [-0.060.26], P0. 23T, rejection rate: RRV 0.5795CI [0.132.49, P0. 46) and the failure rate of transplantation (RRRV 1.7495CI [0.744.07], P0.2c). Conclusion compared with traditional keratoplasty, femtosecond laser assisted keratoplasty can significantly reduce postoperative astigmatism, but can not improve the postoperative best corrected visual acuity.
