贫困聋儿人工耳蜗康复项目服务现状及满意度调查分析

【摘要】:我国0~6岁听力残疾儿童数量为13.7万,每年还将新增加2.3万。人工耳蜗可以帮助患有重度、极重度耳聋的儿童重建听力,加以科学的康复训练,绝大部分聋儿能够正常参与社会生活。然而目前我国听力残疾儿童中70%生活在农村地区,那里经济发展水平、医疗卫生条件和康复服务条件相对落后,许多家庭经济收入微薄,无力承担购人工耳蜗的昂贵费用,因此错过了聋儿康复的最佳时期。 我国于2009年开始实施“中国残联贫困聋儿人工耳蜗抢救性康复项目”。该项目计划从2009年到2011年,为1500名1~5岁重度、极重度聋儿免费植入人工耳蜗,并提供术后康复训练。 本文选取人工耳蜗项目救助对象94名,对基本特征及其家庭情况进行描述,收集接受人工耳蜗项目聋儿及其家庭满意度信息,并对项目服务满意度影响因素进行分析,对我国首次以政府行为为贫困听力残疾儿童提供人工耳蜗植入及康复服务的公共服务项目的服务质量作出评价。结果显示,聋儿以农村户口居多,父母主要是初中文化程度。家庭年人均收入1400元左右,低于全国残疾人2005年平均收入水平和全国平均收入水平。患儿术后6个月平均最大言语识别率为63.29%,比术前提高40.03%;平均语言年龄为1.75岁,比术前提高1.09岁。项目服务的满意度平均值为71.94%,其中医院手术满意度最高,为83%;康复机构服务满意度最低,为59.6%。性别、年龄、耳聋确诊到申报时间、家庭期望值、术后调机次数、产品是否出现故障是贫困聋儿家庭项目服务满意度的主要影响因素。 本文通过项目服务满意度调查及影响因素分析,初步探索设立我国听力残疾儿童救助项目绩效评价中满意度测评的指标和标准,并提出了服务改进措施,为我国在听力残疾儿童救助项目资金投入的可持续性提供科学依据,以便更加合理的制定资源分配政策,逐步推动残疾儿童保障制度的建立。
[Abstract]:The number of children with hearing disabilities aged 0 to 6 in China is 137,000, and a new 23,000 will be added every year. Cochlear implants can help children with severe or extremely severe deafness to reconstruct their hearing and undergo scientific rehabilitation training. Most deaf children can participate in social life normally. However, at present, 70% of the children with hearing disabilities in our country live in rural areas, where the level of economic development, medical and health conditions and rehabilitation services are relatively backward, and many families have a meagre economic income. Unable to afford to buy cochlear implants, missed the best time for deaf children to recover. In 2009, China began to implement the "rescue rehabilitation project for cochlear implants of poor deaf children with disability in China". The project is planned from 2009 to 2011. The project provides free cochlear implant for 1,500 children with severe and extremely severe deafness aged 1 to 5 years, and provides postoperative rehabilitation training. In this paper, 94 cochlear implant recipients were selected to describe the basic characteristics and their families, to collect information on the satisfaction of deaf children and their families, and to analyze the influencing factors of project service satisfaction. This paper evaluates the service quality of the public service project which provides cochlear implant and rehabilitation services for poor hearing disabled children by government behavior for the first time in China. The results show that the majority of deaf children are rural hukou. The parents are mainly junior high school education. The annual per capita income of the family is about 1,400 yuan. It was lower than the average income level of the disabled in 2005 and the national average income level in 2005. The average maximum speech recognition rate in 6 months after operation was 63.29, which was 40.03% higher than that before operation, and the average language age was 1.75 years old. The average degree of satisfaction of the project service was 71.94 years higher than that of the pre-operation. The hospital had the highest satisfaction degree of operation with 830.The rehabilitation institution had the lowest satisfaction degree of 59.6. gender, age, the time to declare the hearing loss, family expectation, gender, age, hearing loss, the time to declare the deafness, and the expectation of the family. The frequency of postoperative machine adjustment and the failure of the product are the main factors influencing the service satisfaction of poor deaf children. Through the investigation of project service satisfaction and the analysis of influencing factors, this paper preliminarily explores the establishment of indicators and standards of satisfaction evaluation in the performance evaluation of hearing disabled children's rescue projects in China, and puts forward some service improvement measures. This paper provides a scientific basis for the sustainability of the funds invested in the project for the relief of children with hearing disabilities in China, so as to make a more reasonable policy of resource allocation and gradually promote the establishment of a guarantee system for children with disabilities.
