三七总皂苷局部治疗兔眼睑球结膜下出血的效果研究

【摘要】:[目的] 建立兔眼结膜下出血的模型,设立衡量结膜下出血的疗效指标及定量指标,探讨三七总皂苷(PNS)的药理作用,证实三七总皂苷滴眼剂局部治疗兔眼结膜下出血的可行性和有效性。 [方法] 选取健康成年新西兰大白兔20只(40眼),分成用药组和空白对照两大组,其中用药组按药物浓度不同又分为高浓度组、中浓度组、低浓度组,每组5只(10眼),采用自体静脉血结膜下定量注射的方法,建立结膜下出血模型。建模后对用药组给予三七总皂苷滴眼剂治疗,其中低浓度组用0.5%三七总皂苷滴眼剂,中浓度组为1%,高浓度组为2%,待结膜下出血治愈后停药,空白对照组不给予任何治疗。建模后每天裂隙灯显微镜下观察各样本结膜下出血量的变化,并记录治愈时间,设定疗效指标:5d内治愈为显效,6-7d治愈为有效,7d后仍未治愈为无效,按疗效指标统计各组病例数,然后采用SPSS17.0软件进行K-W检验及U检验;同时于建模后1d、3d、5d、7d、9d通过裂隙灯显微拍摄各样本16倍结膜像,采用Image-Pro Plus (IPP)专业图形处理软件对其分析处理,测算结膜下出血区域的面积(Area)、平均光密度(MOD)、累计光密度(IOD)作为定量指标,然后采用SPSS17.0软件进行重复测量的方差分析。通过以上两种方法衡量三七总皂苷局部治疗兔眼结膜下出血的效果。 [结果] 1.以治愈时间作为衡量标准:用药各组(低浓度组、中浓度组、高浓度组)的平均治愈时间小于空白对照组,其总有效率也明显高于空白对照组,且与用药浓度呈正比;各组的疗效差异有统计学意义(P0.05)。进一步两两比较结果显示:疗效在低浓度与空白对照组间、中浓度组与空白对照组间、高浓度组与空白对照组间、低浓度组与高浓度组间,差异有统计学意义(P0.008);疗效在低浓度组与中浓度组间、中浓度组与高浓度组间,差异无统计学意义(P0.008)。 2.以出血面积(Area)作为定量指标:各组Area值均随着时间延长呈降低趋势,其中高浓度组降低的趋势最为显著;Area在各组不同时相的差异有显著性(P0.05)。组间两两比较结果显示:Area在用药各组与空白对照组比较,差异有统计学意义(P0.05),但是用药组各组(高浓度组、中浓度组、低浓度组)之间比较,差异无统计学意义(P0.05)。 3.以平均光密度(MOD)作为定量指标:各组MOD均随着时间延长呈降低趋势,其中高浓度组降低的趋势最为显著;MOD在各组不同时相的差异有显著性(P0.05)。组间两两比较结果显示:MOD在低浓度组与空白对照组间、中浓度组与高浓度组间,差异无统计学意义(P0.05),其余组间两两比较,差异均有统计学差异(P0.05) 4.以累积光密度(IOD)作为定量指标:各组IOD均随着时间延长呈降低趋势,其中高浓度组降低趋势最为显著;IOD在各组不同时相的差异有显著性(P0.05)。组间两两比较结果显示:IOD在低浓度组与高浓度组间、中浓度组与空白对照组间、高浓度组与空白对照组间,差异有统计学意义(P0.05);其余组间两两比较,差异无统计学意义(P0.05)。 [结论] 1.运用结膜下定量注射自体静脉血的方法,可成功建立结膜下出血的动物模型,且可统一模型的出血量,使实验处理因素标准化。 2.采用PNS滴眼剂治疗兔眼结膜下出血,观察比较各组治愈时间,并定量比较各组结膜下出血的Area、MOD、IOD,证实PNS滴眼剂可加快结膜下出血的吸收,使出血面积显著缩小,出血区域颜色变浅,缩短病程,且其疗效与用药浓度呈正比。 3.运用IPP分析处理裂隙灯显微镜下采集的结膜像,测算出血区域Area、 MOD、IOD,作为衡量结膜下出血的定量指标,更为客观有效。其中IOD既考虑了出血面积的大小,又反映了出血区域血色的深浅,较之Area和MOD更全面的测定结膜下血量,IOD是衡量结膜下出血量的最优指标。
[Abstract]:[Objective]
A rabbit model of subconjunctival hemorrhage was established. A set of indicators and quantitative indicators for measuring subconjunctival hemorrhage were set up to investigate the pharmacological effects of 37 total saponins (PNS). It was proved that 37 total saponins eye drops were effective and effective in the treatment of rabbit subconjunctival hemorrhage.
[method]
Healthy adult 20 New Zealand white rabbits (40 eyes), divided into treatment group and control group two, the treatment group according to different drug concentrations and divided into high concentration group, middle concentration group, low concentration group, 5 rats in each group (10 eyes), using the method of quantitative injection of autologous venous blood under the conjunctiva. The establishment of subconjunctival hemorrhage model. After modeling of total saponins from 37 eyedrops in treatment of treatment group, the low concentration group of total saponins from 37 eye drops by 0.5%, the concentration of group was 1%, the high concentration group was 2%, the cure for subconjunctival hemorrhage after drug withdrawal, blank control group was not given any treatment. The amount of bleeding of the sample under the conjunctiva observation day after modeling the slit lamp microscope, and record the cure time, set the efficacy index: 5D cure is effective, cure 6-7d effectively, 7d has not been cured as invalid, according to the efficacy index statistics the number of each group, and then using SPSS17.0 software K-W Test and U test; at the same time to model after 1D, 3D, 5D, 7d, 9D by slit lamp microscopic photography of each sample is 16 times as the conjunctiva (IPP), Image-Pro Plus professional graphics software for the analysis, regional estimates of subconjunctival hemorrhage area (Area), the average optical density (MOD), total the optical density (IOD) as a quantitative index, and then repeated measure ANOVA using SPSS17.0 software. Through the above two methods to measure the effect of total saponins of 37 local treatment of rabbit subconjunctival hemorrhage.
[results]
1. time to heal as a measure of the standard: the treatment group (low concentration group, middle concentration group, high concentration group) the average cure time is less than the blank control group, the total efficiency was significantly higher than the control group, and the drug concentration is proportional to the difference was statistically significant; the effect of (P0.05). A further 22 comparison the results showed that the curative effect in low concentration and the blank control group, middle concentration group and blank control group, high concentration group and blank control group, low concentration group and high concentration group, the difference was statistically significant (P0.008); the curative effect in the low concentration group and middle concentration group, middle concentration group and the high concentration group, the difference was not statistically significant (P0.008).

4. with the integrated optical density (IOD) as a quantitative index: IOD in both groups decreased with time, the high concentration group decreased most significantly; IOD groups in different phases with significant difference (P0.05) between the 22 groups was compared. The results showed that IOD in low concentration group and high concentration group and in the concentration group and blank control group, high concentration group and blank control group, the difference was statistically significant (P0.05); the comparison between the 22 groups, the difference was not statistically significant (P0.05).
[Conclusion]
1., using the method of quantitative injection of autologous venous blood under conjunctiva, we can successfully establish animal models of subconjunctival hemorrhage, and we can unify the amount of bleeding and standardize the experimental factors.
2. using PNS eyedrops in treatment of rabbit subconjunctival hemorrhage, observed the healing time, Area, hemorrhage and quantitative comparison of subconjunctival MOD, IOD PNS, confirmed that the eye drops can accelerate the absorption of subconjunctival hemorrhage, the hemorrhage area reduced significantly, the bleeding area pale, shorten the course, and the the efficacy and drug concentration.
3. by IPP analysis under the slit lamp microscope and collected conjunctival like, measurement of hemorrhagic area Area, MOD, IOD, as a measure of subconjunctival hemorrhage, which is more objective and effective. IOD can account for the bleeding area, but also reflects the depth of blood bleeding area, subconjunctival blood volume determination is more comprehensive the Area and MOD, IOD is the optimal index measure of subconjunctival bleeding.
