Twist、E-cadherin及N-cadherin蛋白在声门上型喉癌组织中的表达及意义

【摘要】:目的:检测声门上型喉癌组织及声带息肉组织标本中的Twist、 E-cadherin和N-cadherin蛋白表达,探讨以上三种蛋白在喉部良恶性组织中的差异性表达及其与声门上型喉癌患者临床病理特征的关系,以及三种蛋白表达相关性。结合临床随访资料,分析上述三种蛋白表达与声门上型喉癌预后之间的关系。 方法:采用免疫组化方法检测46例声门上型喉癌组织及30例声带息肉组织中的Twist、E-cadherin及N-cadherin蛋白的表达。 结果: 1.Twist、E-cadherin及N-cadherin蛋白在声门上型喉癌的阳性表达率分别是69.57%、39.13%、50.00%,在声带息肉组织中分别为36.67%、86.67%、13.33%,以上三种蛋白在喉部良恶性组织中的差异有统计学意义(p0.05)。 2.低分化、淋巴结阳性、T3-4、中晚期的声门上型喉癌组织中的Twist蛋白和N-cadherin蛋白的阳性表达率高于在中高分化、淋巴结阴性、T1-2、早期的声门上型喉癌组织中的阳性表达率(p0.05)。低分化、淋巴结阳性、T3-4、中晚期的声门上型喉癌中的E-cadherin蛋白阳性表达率低于在中高分化、淋巴结阴性、T1-2、早期的声门上型喉癌组织的阳性表达率(p0.05)。 3.相关分析示:在声门上型喉癌中Twist蛋白表达与E-cadherin蛋表达负相关(r=-0.535,p=0.000);而Twist蛋白表达则与N-cadherin蛋白表达正相关(r=0.378,p=0.010);E-cadherin蛋白表达与N-cadherin蛋白表达呈负相关(r=-0.445,p=0.002)。 4. Twist蛋白及N-Cadherin蛋白阳性表达组的3年局部复发、颈淋巴结复发高于阴性组(p0.05),E-Cadherin蛋白阳性表达组的3年局部复发、颈淋巴结复发低于阴性组(p0.05)。 5. Twist蛋白、N-cadherin表达阳性的3年总OS及DFS均显著低于阴性组(p0.05)。E-cadherin表达阳性的3年OS及DFS均显著高于阴性组(p0.05)。 6.Cox回归模型多因素分析示:肿瘤浸润深度及淋巴结转移情况是影响声门上型喉癌患者3年OS的独立预后因素(HR分别为8.639、4.276,p值分别为0.037、0.023)。 结论: 1.Twist、 E-cadherin及N-cadherin蛋白与声门上型喉癌的发生有关。 2.Twist、E-cadherin及N-cadherin与声门上型喉癌的侵袭、转移等有关。 3.Twist蛋白可能与声门上型喉癌中上皮间质转化的发生有关。 4.Twist、E-cadherin与N-cadherin蛋白可能与声门上型喉癌预后有关。
[Abstract]:Objective: to detect the expression of Twist, E-cadherin and N-cadherin proteins in supraglottic laryngeal carcinoma and vocal cord polyp tissues, and to explore the relationship between the expression of these three proteins in benign and malignant laryngeal tissues and the clinicopathological features of supraglottic laryngeal carcinoma. And three kinds of protein expression correlation. To analyze the relationship between the expression of these three proteins and the prognosis of supraglottic laryngeal carcinoma. Methods: the expression of Twister-E-cadherin and N-cadherin protein in 46 cases of supraglottic laryngeal carcinoma and 30 cases of vocal cord polyp were detected by immunohistochemical method. Results: 1. The positive expression rates of Twister-E-cadherin and N-cadherin protein in supraglottic laryngeal carcinoma were 69.57and 39.1350.The positive rates of Twister-E-cadherin and N-cadherin protein were 39.57and 39.130.The positive rates of Twister-E-cadherin and N-cadherin protein in vocal cord polyps were 36.670.67 and 13.33g, respectively. There was a significant difference among the above three proteins in benign and malignant larynx tissues (p 0.05). 2. The positive expression rate of Twist protein and N-cadherin protein in advanced supraglottic laryngeal carcinoma was higher than that in middle and high differentiation, lymph node negative T1-2, and early supraglottic laryngeal carcinoma (p0.05). The positive expression rate of E-cadherin protein in advanced glottic laryngeal carcinoma was lower than that in middle and high differentiation, lymph node negative tumor T1-2, and the positive expression rate of p0.05in early supraglottic laryngeal carcinoma. 3. Correlation analysis showed that the expression of Twist protein was negatively correlated with the expression of E-cadherin eggs in supraglottic laryngeal carcinoma, while the expression of Twist protein was positively correlated with the expression of N-cadherin protein. There was a negative correlation between the expression of E-cadherin protein and the expression of N-cadherin protein in supraglottic laryngeal carcinoma. 4. The local recurrence of Twist protein and N-Cadherin protein positive group was 3 years, and the recurrence of cervical lymph node was higher than that of negative group. The recurrence of cervical lymph node was lower than that of negative group. 5. The expression of Twist protein N-cadherin was significantly lower in the 3-year total OS and DFS than that in the negative group. The expression of DFS and OS in the negative group was significantly higher than that in the negative group. The multivariate analysis of 6.Cox regression model showed that the depth of tumor invasion and lymph node metastasis were independent prognostic factors for the patients with supraglottic laryngeal carcinoma for 3 years. The HR was 8.639 卤4.276 (P = 0.037) and 0.023 ~ (th), respectively. Conclusion: 1. Twist. E-cadherin and N-cadherin proteins were associated with supraglottic laryngeal carcinoma. 2. Twistoma E-cadherin and N-cadherin were associated with the invasion and metastasis of supraglottic laryngeal carcinoma. The 3.Twist protein may be associated with epithelial interstitial transformation in supraglottic laryngeal carcinoma. 4. Twistus E-cadherin and N-cadherin protein may be related to the prognosis of supraglottic laryngeal carcinoma.
