糖尿病视网膜病变视网膜中央动脉血流动力学变化

【摘要】: 目的:通过研究糖尿病视网膜病变不同时期视网膜中央动脉血流动力学变化,为糖尿病视网膜病变的诊断及治疗监测及预后评估提供检测依据。 方法:利用彩色多普勒超声诊断仪(Color Doppler Imaging, CDI)测定62例(105眼)糖尿病视网膜病变(diabetic rctinopathy,DR)患者及20例正常对照组球后视网膜中央动脉(central retinal artery, CRA)血流状态,检测指标包括视网膜中央动脉(CRA)的最大收缩期血流速度(peak systolic velocity, PSV)、舒张末期血流速度(end diastolic velocity,EDV)、平均血流速度(mean velocity,Vm)、搏动指数(pulsatility index,PI)和阻力指数(resistant index,RI)。分组如下:①对照组(normal control, NC选取健康人20例(40眼)作为对照。②糖尿病组依据最新国际分期接糖尿病视网膜病变的严重程度,分为三组研究组,其中包括糖尿病无视网膜病变(nondiabetic retinopathy. NDR)36眼,糖尿病视网膜病变非增殖期(nonproliferative diabetic retinopathy, NPDR) 34眼,糖尿病视网膜病变增殖期(proliferative diabetic retinopathy, PDR)35眼。各组之间年龄、性别差异无统计学意义(P0.05),采用成组t检验统计学方法对上述检测指标进行各组之间分析比较 结果:结果显示①糖尿病各组CRA的PSV、EDV和Vm均较正常对照组(NC组)减低(P0.05),RI和PI较正常对照组增高(P0.05)。②NPDR组和PDR组患者CRA的PSV、EDV及Vm较NDR组降低而RI和PI则增高(P0.05)。③NPDR与PDR之闻各参数差异无统计学意义(P0.05)。 结论:通过CDI对2型DM患者球后视网膜中央动脉血流动力学变化的动态检测,对DR的早期诊断、治疗监测及预后评估有一定临床应用价值。
[Abstract]:Objective: to provide the basis for the diagnosis and treatment monitoring and prognosis evaluation of diabetic retinopathy by studying the hemodynamic changes of central retinal artery in different stages of diabetic retinopathy.
Methods: the blood flow status of the central retinal artery (central retinal artery, CRA) in 62 patients (105 eyes) with diabetic retinopathy (diabetic rctinopathy, DR) and 20 normal controls was measured by Color Doppler Imaging (CDI). The measurement index included the maximum contraction of the central retinal artery (CRA). Peak systolic velocity (PSV), end diastolic blood velocity (end diastolic velocity, EDV), average velocity of blood flow (mean velocity, Vm), pulsatile index, and resistance index were grouped as follows: 20 healthy persons (40 eyes) were selected as control. According to the severity of the latest international stage of diabetic retinopathy, the urinary disease group was divided into three groups, including 36 eyes of diabetic retinopathy (nondiabetic retinopathy. NDR), 34 eyes of non proliferative diabetic retinopathy (nonproliferative diabetic retinopathy, NPDR), and diabetic retinopathy proliferation (prol). Iferative diabetic retinopathy, PDR) 35 eyes. There was no statistically significant difference in age and sex between each group (P0.05). A group of t test statistics were used to analyze and compare the above indexes.
Results: the results showed that the PSV, EDV and Vm of CRA in each group were lower than that of the normal control group (P0.05), and the RI and PI were higher than those of the normal control group (P0.05).
Conclusion: the dynamic changes of the hemodynamic changes of the central retinal artery in type 2 DM patients by CDI have some clinical value for the early diagnosis of DR, the monitoring of the treatment and the evaluation of the prognosis.
