内眼手术患者术前不同措施预防结膜囊感染病原学分析

【摘要】：目的比较分析内眼手术患者术前使用抗菌药物滴眼联合结膜囊冲洗及全身使用抗菌药物的结膜囊无菌化效果。方法随机选择2012年4月-2014年2月收治的内眼手术100眼,分为对照组25眼及试验组75眼,试验组随机分为3组,试验组和对照组患者均在未干预前实施结膜囊细菌培养;对照组患者均采用生理盐水进行冲洗,试验1组采用左氧氟沙星滴眼、试验2组采用左氧氟沙星滴眼联合生理盐水冲洗、试验3组采用左氧氟沙星滴眼、生理盐水冲洗联合全身抗菌药物静脉滴注;两组患者均于用药后第3天对结膜囊采样进行细菌培养,采用SPSS15.0统计软件进行分析。结果对照组及3个试验组采用感染预防措施前检出率均为36.0%,实施感染预防措施后,病原菌检出率对照组为24.0%、试验1组为12.0%、试验2组为8.0%、试验3组为4.0%,组间差异有统计学意义(P0.05);干预后结膜囊细菌培养阳性呈现明显递减趋势。结论内眼术前使用左氧氟沙星滴眼液并采用生理盐水进行结膜囊冲洗,可有效预防眼内感染,术前1h静脉使用左氧氟沙星注射液可提高结膜囊细菌清除率。
[Abstract]:Objective to compare and analyze the aseptic effect of intraocular surgery with antiseptic drops combined with irrigation of conjunctival sac and systemic use of antimicrobial agents. Methods from April 2012 to February 2014, 100 eyes were randomly divided into control group (25 eyes) and experimental group (75 eyes). The experimental group was randomly divided into 3 groups. The patients in the experimental group and the control group were treated with bacterial culture of conjunctival sac before intervention. The patients in the control group were washed with normal saline, the eyes of group 1 were treated with levofloxacin, the eyes of group 2 were washed with levofloxacin combined with normal saline, and the eyes of group 3 were treated with levofloxacin. The bacterial culture of conjunctival sac was carried out on the 3rd day after treatment, and the results were analyzed by SPSS15.0 software. Results the positive rate of infection prevention was 36.0 in the control group and in the three experimental groups. The detection rate of pathogenic bacteria was 24.0 in control group, 12.0 in test 1, 8.0 in test 2 and 4.0 in test 3. The positive rate of bacterial culture in conjunctival sac showed a decreasing trend after intervention. Conclusion Levofloxacin eye drops and saline for conjunctival sac irrigation can effectively prevent intraocular infection. The bacterial clearance rate of conjunctival sac can be improved by intravenous levofloxacin injection 1 hour before operation.
