手术治疗对儿童睡眠障碍性呼吸患者生活质量的影响

【摘要】：目的:通过手术切除肥大的腺样体和/或扁桃体治疗由于腺样体和/或扁桃体肥大而引起的儿童睡眠障碍性呼吸(SDB),探讨手术治疗对儿童SDB患者生活质量的影响。方法:对30例因扁桃体和/或腺样体肥大的SDB患者,采用儿童OSAHS疾病特异性生活质量调查(OSA-18)法,在术前一周内和术后6个月进行问卷式调查,通过手术治疗前后患儿生活质量的变化,评价手术治疗对SDB患者生活质量的影响。结果:其中单行腺样体切除6例,单行双侧扁桃体切除2例,行双侧扁桃体切除+腺样体切除22例;SDB患儿术后生活质量总评及各维度评分较术前明显下降(P均0.01),睡眠障碍、身体症状及对监护人的影响这三个方面均得到了显著改善;手术前有43.33%的患儿生活质量受到严重影响,手术后有63.33%的患儿生活质量得到明显改善。结论:儿童SDB严重影响患儿的生活质量,扁桃体和/或腺样体切除术是有效的治疗方法,OSA-18评分可作为儿童SDB临床辅助诊断和疗效观察的评价指标。
[Abstract]:Objective: To investigate the effects of surgical treatment on the quality of life of children with SDB by surgical resection of hypertrophic adenoids and / or tonsillar in children with adenoids and / or tonsillar hypertrophy (SDB).
Methods: 30 cases of SDB patients with tonsillar and / or adenoid hypertrophy were investigated with a questionnaire survey of OSAHS disease specific quality of life (OSA-18), a week before and 6 months after the operation. The effects of hand operation on the quality of life of SDB patients were evaluated before and after the operation.
Results: there were 6 cases of unilateral adenoidectomy, 2 cases of unilateral tonsillectomy and 22 cases of bilateral tonsillectomy and adenoidectomy. The total evaluation of quality of life after operation in SDB children was significantly lower than before operation (P 0.01), sleep disorders, physical symptoms and influence on guardianship, all of the three aspects were significantly improved; before operation 43.33% of the children were severely affected by the quality of life, and 63.33% of them had a marked improvement in their quality of life.
Conclusion: SDB in children seriously affects the quality of life of children. Tonsillectomy and / or adenoidectomy is an effective treatment. The OSA-18 score can be used as an evaluation index for the clinical diagnosis and observation of SDB in children.
