氧化应激上调IL-17在Graves眼病的研究

【摘要】:目的 研究细胞因子白细胞介素17(interleukine-17,IL-17)在Graves眼病(Graves' ophthalmopathy,GO)发病中的作用,并探讨氧化应激与IL-17的关系。 方法 25例初发活动期的Graves眼病病人作为GO组,15例健康人作为对照组纳入研究。临床评估GO病人眼病活动度(clinical activity score,CAS)及严重度(NOSPECS))。每位入选者均采集外周血,检测血清中游离三碘甲腺原氨酸(free triiodothyronine,FT3 )、游离四碘甲腺原氨酸(free tetraiodothyronine,FT4)、超敏促甲状腺激素(hypersensitive thyroid-stimulating hormone,μTSH)、甲状腺球蛋白抗体(thyroglobulin antibody,TgAb)、甲状腺过氧化物酶抗体(thyroid peroxidase antibody,TPOAb)、促甲状腺激素受体抗体(thyrotrophin receptor antibody ,TRAb)以及IL-17、丙二醛(monohydrate ,MDA)水平和超氧化物歧化酶(superoxide dismutase ,SOD)活力。 从对照组和GO组中各选取10例,采外周血。分离T淋巴细胞,体外与过氧化氢(Hydrogen Peroxide Solution,H2O2)和/维生素C(Vitamin C,Vitc)共同培养。每例标本的T淋巴细胞均分组为:空白组(H2O2 =0,VitC =0),过氧化氢组(H_2O_2 =1 000 nM,VitC=0),维生素C组(H_2O_2 =0,VitC =25μM),过氧化氢+维生素C组(H_2O_2 =1000 nM,VitC=25μM),检测细胞培养上清液IL-17水平,并进行统计分析。 结果 1.GO病人眼病活动度评分均≥3分(范围3-6,均值4.88±0.93),严重度NOSPECS≥3级(范围3-5,均值3.56±0.65)。 2.与对照组相比,GO组血清FT3(13.49±4.96 vs. 3.11±0.16 pg/ml),FT4(6.03±2.64 vs.1.24±1.36 ng/dl),TPOAb(300.38±233.05 vs.44.20±37.51 IU/ml),TgAb(1542.54±1614.51 vs.59.52±34.66 IU/ml),TRAb(19.41±3.77 vs.12.47±1.15U/L)及IL-17(13.62±2.43 vs. 10.15±0.93ng/L)水平较正常显著升高(P㩳0.01或P㩳0.05),μTSH (0.02±0.02 vs.1.24±0.14 mIU/L)显著降低(P㩳0.01),年龄无显著差异(30.00±5.30 vs.31.04±8.33岁)(P㧐0.05)。GO血清IL-17与眼病临床活动评分CAS(r=0.43,P=0.03)正相关,与严重度无相关。 3.GO组血清MDA较对照组(6.77±1.11 vs. 4.16±0.61nmol/ml)显著升高(P㩳0.05),SOD活力(53.95±17.53 vs. 73.16±15.33U/ml)显著降低(P㩳0.05)。GO组血清MDA水平与IL-17呈显著正相(r=0.490,P=0.046);MDA水平、SOD活力与眼病临床活动评分和严重度均无相关性(P㧐0.05)。 4 T淋巴细胞培养上清液IL-17水平检测分析,GO组中T淋巴细胞的H2O2组(27.55±4.75 ng/L)㧐空白组(20.24±2.13 ng/L )㧐VitC(18.76±1.13 ng/L)组及H2O2+VitC(17.59±1.85 ng/L)组(P㩳0.05);VitC组及H2O2+VitC组间无显著差异(P㧐0.05)。对照组的T淋巴细胞各组间无显著差异(P㧐0.05)。 结论 1.初发活动期GO病人外周血IL-17显著升高,并与CAS相关,可能参与了GO的发病; 2.氧化应激参与了GO病人IL-17水平的上调。VitC可在一定程度减少IL-17释放,为临床补充抗氧化剂作为GO辅助治疗提供思路和依据。
[Abstract]:Objective to investigate the role of interleukine-17 (IL-17) in the pathogenesis of Graves' ophthalmopathy (Graves' ophthalmopathy go) and to explore the relationship between oxidative stress and IL-17. Methods 25 patients with Graves' ophthalmopathy in the first active stage were included in the study as the go group and 15 healthy persons as the control group. T lymphocytes were isolated and co-cultured with hydrogen peroxide (H _ 2O _ 2) and vitamin C (Vitamin C) in vitro.
