室带位置对发音影响的研究

【摘要】:目的:提出室带异常的分度;应用XION嗓音分析软件,对室带处于不同位置时的发声情况进行声学测试,探讨室带处于不同位置时的嗓音变化特征;探讨室带与发音的关系及室带的位置对发音的影响。 方法:应用多媒体技术,采用XION声学分析软件,选择2009年2月到2010年12月于天津医科大学第二医院耳鼻喉科就诊患者100例,其中室带中间位20例,Ⅰ度外展位20例,Ⅱ度外展位20例,Ⅰ度内收位20例,Ⅱ度内收位20例。对上述100例患者行电子喉镜和嗓音声学分析。利用XION电子喉镜进行室带位置的确定,位置确定后,对受试者进行声音疲劳试验指数及声音障碍指数测定,声音疲劳试验中测得的参数为低声压级和高声压级的错误率,对同一室带位置的男女之间的声压级的错误率进行比较,并对不同位置室带声压级的错误率进行比较;声音障碍指数测定中所得到的参数为最高基频、最小音量、最长发音时间、Jitter、DSI,比较同一室带位置的男女之间的各参数有无统计学意义,并对室带不同位置的同一参数进行比较。 结果:声音疲劳试验测试:①组内性别间比较:室带Ⅱ度外展位男女性别间比较均无统计学意义(P0.05);室带Ⅰ度外展位男女性别间比较均无统计学意义(P0.05);室带中间位低声压级男女性别间比较有统计学意义(P0.05),高声压级男女性别间比较无统计学意义(P0.05);室带Ⅰ度内收位男女性别间比较无统计学意义(p>0.05);室带Ⅱ度内收位低声压级男女性别间比较无统计学意义(P0.05),高声压级男女性别间比较有统计学意义(P0.05)。②室带不同位置同一参数比较:男性各组间低声压级错误率比较:P均0.05;男性高声压级错误率室带Ⅰ度外展位和Ⅱ度外展位之间比较明显无差异(P0.05),余有统计学意义(P0.05);女性室带Ⅰ度内收位和Ⅱ度内收位间之间低声压级错误率比较无统计学意义(p0.05),室带Ⅰ度外展位和Ⅱ度外展位之间低声压级错误率比较无统计学意义(p0.05),余有统计学意义(p0.05);女性高声压级错误率比较:室带Ⅱ度外展位和室带Ⅰ度内收位比较无统计学意义(P0.05),余有统计学意义(p0.05)。 声音障碍指数测试:①组内性别间比较:室带Ⅱ度外展位男女之间最高基频、最小音量、最长发音时间、Jitter比较无统计学意义(p0.05),DSI比较有统计学意义(P0.05);Ⅰ度外展位男女之间最高基频、最小音量、最长发音时间、Jitter、DSI各值比较无统计学意义(P0.05);中间位男女之间最高基频、最小音量、最长发音时间、Jitter、DSI各值比较无统计学意义(P0.05);Ⅰ度内收位男女之间最高基频、最小音量、最长发音时间、Jitter、DSI各值比较无统计学意义(P0.05);Ⅱ度内收位男女之间最高基频、最小音量、Jitter、DSI各值比较无统计学意义(P0.05),最长发音时间比较有统计学意义(P0.05)。②室带不同位置同一参数比较的结果:男性、女性室带各位置DSI比较均有统计学意义(P0.05);男性Ⅱ度外展位和Ⅰ度内收位最高基频比较无统计学意义(P0.05),其他位置最高基频比较有统计学意义(P0.05),女性Ⅱ度外展位和Ⅰ度内收位最高基频比较无统计学意义(P0.05),其他位置最高基频比较有统计学意义(P0.05);男性、女性各位置之间最小音量比较均无统计学意义(P0.05);男性、女性各位置最长发音时间比较均有统计学意义(P0.05);男性室带Ⅱ度外展位和Ⅰ度外展位Jitter比较无统计学意义(P0.05),其余位置Jitter均比较有统计学意义(P0.05),女性室带Ⅱ度外展位和Ⅰ度外展位、室带Ⅱ度外展位和Ⅰ度内收位Jitter比较无统计学意义(P0.05),其他位置比较均有统计学意义(P0.05)。 结论:①室带参与了发音的过程;②室带在正常位置时,声音疲劳度和声音障碍指数最低,室带偏离正常位置后,声音疲劳度和声音障碍指数增高;随偏离的程度的加大声音疲劳度和声音障碍指数加大;③室带内收位对声音的影响和室带外展位对声音的影响有差异,病变不具有对称性,室带内收位声音疲劳的程度和声音障碍指数的程度高于室带外展位,Ⅱ度内收位对发音影响最大;④声音疲劳测试和声音障碍指数测试可以作为评价室带病变的重要客观指标。
