散瞳饮水暗室低头激发试验在可疑青光眼中的临床应用

【摘要】: 目的 观察并评价散瞳饮水暗室低头激发试验(Mydriasis water-drinking dark-room lower-head-position provocative test, MWDLPT, PT)在可疑青光眼(Glaucoma suspects, GS)中的临床应用。 方法 收集2004年2月~2010年1月间于中国医大一院眼科门诊青光眼组就诊的GS共37例(74眼),年龄15~78岁(平均年龄50.7±19.3岁)。非同日分别行PT和日间眼压(Daytime intraocular pressure, DIOP)测量等辅助检查,分别记录两种方法测得的眼压波动值(AIOP)和眼压峰值(Peak IOP),两种方法均以AIOP≥8mmHg为阳性,比较两种方法参数间的关系。分别比较PT结果与周边前房深度(Peripheral anterior chamber depth, PACD)和年龄(Age)间的关系。采用X2检验及线性回归分析进行统计学分析,以P0.05表示有统计学意义。 结果 PT阳性者为21例(37眼)。无1例出现严重的并发症(如明显的眼痛,头痛,视力下降及恶心呕吐等)。所有PT试验对象的眼压在试验后1-2小时内降至基线水平,部分病例应用了缩瞳剂。PT与DIOP两种方法的Peak IOP成显著的正相关关系,且PT法的阳性率显著高于DIOP法。在PACD≤1/4角膜厚度(Corneal thickness, CT)组PT的阳性率显著高于PACD>1/4CT组。PT法△IOP与年龄无相关性。 结论 PT可作为一种新的可疑青光眼的激发试验,在对眼压波动情况的检查上优于DIOP法,可能更适用于闭角型青光眼(Angle-closure glaucoma, ACG),且不受年龄影响,应用范围较广泛,是一种相对安全有效的检查方法。
