23G微创玻璃体切除手术治疗球形视网膜脱离的疗效

【摘要】：目的:探讨23G微创玻璃体切除手术治疗球形视网膜脱离的疗效及其安全性。方法:2010年1月至2013年12月在我院就诊的36例(36眼)球形视网膜脱离病例,接受23G微创玻璃体切除手术,术后随访裂隙灯、眼压、最佳矫正视力、间接检眼镜检查和并发症发生情况。结果:随访时间9~18(12.7±2.1)个月,术后视网膜解剖复位率97.2%(35/36眼),1眼在术后视网膜未完全复位,2周后下方发现新裂孔,行巩膜外扣带术后视网膜复位。末次随访最佳矫正视力显著高于术前,除术中手术切口渗漏缝合6眼(16.7%),术后一过性眼压升高9眼(25.0%),前房少量积血1眼(2.8%),晶状体羽毛状混浊5眼(13.9%)外,未出现医源性裂孔、脉络膜脱离、眼内炎等严重并发症。结论:23G微创玻璃体切除术治疗球形视网膜脱离是安全有效的。
[Abstract]:Objective: to evaluate the efficacy and safety of 23 G minimally invasive vitrectomy in the treatment of spherical retinal detachment. Methods: from January 2010 to December 2013, 36 patients (36 eyes) with spherical retinal detachment underwent 23 G minimally invasive vitrectomy, followed up by slit lamp, intraocular pressure, and best corrected visual acuity. Indirect ophthalmoscope examination and complications. Results: the follow-up time was 9 months (12.7 卤2.1) months. The anatomical reattachment rate of retina was 97.2% (35 / 36 eyes). Retinal reattachment was performed after scleral buckling. The best corrected visual acuity at the last follow-up was significantly higher than that before operation, except for 6 eyes (16.7%) with operative incision leakage and suture, 9 eyes (25.0%) with postoperative transient intraocular pressure elevation, and 1 eye (2.8%) with a small amount of hyphema in the anterior chamber. There were no serious complications such as iatrogenic hiatus, choroidal detachment and endophthalmitis in 5 eyes (13.9%) of lens feather opacity. Conclusion: 23 G minimally invasive vitrectomy is safe and effective in the treatment of spherical retinal detachment.
