TGF-β1、TβR-Ⅱ在鼻内翻性乳头状瘤中的表达及其临床意义

【摘要】: 目的 鼻内翻性乳头状瘤(NIP)是一种发生于鼻腔鼻窦粘膜上皮组织的良性肿瘤,占鼻部肿瘤的0.5%-4%,因复发率高和易恶变等特征,目前成为研究热点。通过彻底手术可解决复发的问题,但恶变的机制尚未明确,目前仍缺乏判断NIP恶变倾向的可靠依据。通过对NIP相关因子的深入研究,找到预测NIP恶变的可靠证据,指导临床对高危患者进行重点监测,对提高NIP患者的无瘤生存率是很有必要的。TGF信号转导通路广泛参与细胞的分化、增殖及凋亡等,在头颈肿瘤的发生发展过程中起着重要的作用。TGF-β1、TβR-Ⅱ是TGF信号转导通路的枢纽因子,若其中任一因子失活均可导致TGF信号传导通路中断,生长失控,诱导瘤体形成。本研究旨在探讨TGF-β1及Ⅱ型受体(TβR-Ⅱ)在NIP中的表达及与NIP恶变的关系,为临床提供坚实的理论依据。 方法 回顾性分析72例经病理证实为NIP的患者资料,将其按病理分为NIP良性病变组、NIP生长活跃伴不典型增生组、NIP恶变组,用应用免疫组化检测TGF-β1和TβR-Ⅱ在各组中的表达情况,另以10例同期鼻息肉手术标本做为对照组。 结果 72例患者恶变率是16.67%(12/72),同时恶变率4.17%(3/72),异时恶变率12.50%(9/72)。TGF-β1在NIP恶变组织、NIP良性病变组织、鼻息肉组织中的阳性表达率分别为91.67%、34.09%和30%,NIP恶变组织中的TGF-β1阳性表达高于NIP良性病变组织和鼻息肉组(P0.05);而TβR-Ⅱ在NIP恶变组织中表达缺失,在NIP良性病变组(40.91%)和鼻息肉组(40%)中表达增高,经比较差异有统计学意义(P0.05);TGF-β1在NIP伴不典型增生组阳性表达率(68.75%)与NIP恶变组比较无明显差异(P0.05)。汇总三组NIP, TGF-β1和TβR-Ⅱ表达呈负相关。 结论 TGF-β1、TβR-Ⅱ表达异常与NIP恶变密切相关,但与NIP的发生可能无关;对伴不典型增生NIP进行TGF-β1和TβR-Ⅱ蛋白的联合检测预测NIP恶变提供参考。
