中耳加压治疗豚鼠膜迷路积水及其机制的实验研究

【摘要】:目的 采用激素法制备豚鼠膜迷路积水动物模型[1],将其放入压力舱内进行中耳加压,以探讨中耳加压是否可以抑制膜迷路积水,并通过中耳加压治疗后豚鼠耳蜗水通道蛋白2(aquaporin-2,AQP-2)表达的变化,进一步探讨中耳加压对膜迷路积水影响的机制。 方法 将30只豚鼠随机分为3组,每组10只:对照组不做处理;积水组和加压组腹腔注射醋酸去氨加压素1周(4μg/kg/d),制备膜迷路积水模型。且加压组第2,3周放进压力舱行加压治疗,各组豚鼠生存期都是3周。观察3组动物耳蜗形态和功能的变化;用免疫组化方法检测三组豚鼠耳蜗AQP-2的表达。 结果 (1)积水组听脑干反应(ABR)听阈值为[(58.34±7.79) dB SPL]高于加压组[(33.29±3.26) dB SPL]和对照组[(20.84±2.13)dB SPL](P0.05);(2)膜迷路积水程度:积水组R值为(0.49±0.11)较加压组(0.36±0.06)膜迷路积水为重P0.05);对照组无膜迷路积水。(3)免疫组化实验:AQP-2主要表达在蜗管外侧壁、Corti’s器、螺旋神经节细胞等处,积水组蜗管外侧壁平均灰度值为(134.63±6.64)较对照组(159.36±2.63)、加压组(147.06±4.91)豚鼠AQP-2的表达增加(P0.05)。 结论 1中耳加压可显著减轻由醋酸去氨加压素而引起的豚鼠膜迷路积水并改善了耳蜗功能。 2中耳加压治疗使膜迷路积水豚鼠耳蜗的AQP-2的表达下降,说明中耳加压使膜迷路积水减轻的治疗机制可能与AQP-2的表达下降有关。
[Abstract]:Objective to establish an animal model of membranous labyrinth hydrops in guinea pigs by hormone method, and put it into the pressure chamber for middle ear pressure, so as to explore whether middle ear pressure can inhibit membranous labyrinth hydrops. The changes of aquaporin-2 (aquaporin-2,AQP-2) expression in guinea pig cochlea after middle ear pressure therapy were used to investigate the mechanism of the effect of middle ear pressure on membranous labyrinthine hydrops. Methods Thirty guinea pigs were randomly divided into 3 groups: control group (n = 10), hydrops group (n = 10), hydrolysate group (n = 10) and hydrops group (n = 10), and hydrops model of membranous labyrinth was established by intraperitoneal injection of desaminopressin acetate (4 渭 g/kg/d). In the second week, the guinea pigs in the compression group were put into the pressure chamber for compression therapy, and the survival time of each group was 3 weeks. The changes of cochlea morphology and function in three groups were observed and the expression of AQP-2 in cochlea of guinea pigs was detected by immunohistochemical method. Results (1) the (ABR) threshold of auditory brainstem response in hydronephrosis group [(58.34 卤7.79) dB SPL)] was higher than that in pressure group [(33.29 卤3.26) dB SPL)] and control group (20.84 卤2.13) dB SPL) (P0.05). (2) the degree of membranous labyrinth hydrops: the R value of hydronephrosis group (0.49 卤0.11) was higher than that of pressure group (0.36 卤0.06) (P0.05). (3) Immunohistochemical study: AQP-2 was mainly expressed in the lateral wall of cochlear duct, Corti's organ, spiral ganglion cells and so on. The mean gray value of the lateral wall of the cochlear canal in the hydrops group was (134.63 卤6.64) higher than that in the control group (159.36 卤2.63), and the expression of AQP-2 in the pressurized group (147.06 卤4.91) was increased (P0.05). Conclusion 1 the middle ear pressure can significantly reduce the hydrocephalus caused by deaminopressin acetate and improve the cochlea function in guinea pigs. 2the expression of AQP-2 in cochlea of guinea pigs with membranous hydrolabyrinth decreased after middle ear pressure treatment, indicating that the mechanism of reducing membranous hydrolabyrinth by middle ear pressure may be related to the decrease of AQP-2 expression.
