调强放疗联合爱必妥治疗局部复发鼻咽癌的临床分析并文献复习

【摘要】： 目的：探讨局部复发鼻咽癌的临床特点、治疗方法以及调强放疗联合爱必妥治疗局部复发鼻咽癌的疗效和安全性。资料及方法：广西医科大学第一附属医院2007年12月至2009年10月收治的3例接受IMRT联合爱必妥治疗的局部复发鼻咽癌的临床特点：(1)3例患者均为女性,年龄分别为38岁、53岁和39岁；(2)3例患者初治分期均为T3N0M0,1例行常规放疗后7年复发；2例行调强放疗后1年内复发；复治分期分别为rT4N0M0、rT4N0M0、rT4N1aM0(3)治疗方法：2例患者采用7野调强,1例放疗剂量GTV58Gy, CTV48Gy,后去外院行赛博刀治疗,1例放疗剂量GTV68Gy, CTV 58Gy,另1例采用9野调强,GTV70Gy, CTV 60Gy; (7)3例患者均联合爱必妥分子靶向治疗,放疗前1周开始用药,第1周400mg／m2,从第2周开始250 mg／m2,至放疗结束。结果：3例患者均能耐受治疗,短期内复查MRI,病灶均较治疗前明显好转,颅神经症状明显改善。结论：调强放疗联合分子靶向治疗可改善局部复发鼻咽癌患者的局部症状,近期疗效满意,毒副反应可耐受。
[Abstract]:Objective: to investigate the clinical features, treatment methods and efficacy and safety of intensity-modulated radiotherapy (IMRT) combined with Ebitol in the treatment of locally recurrent nasopharyngeal carcinoma (RNPC). Materials and methods: from December 2007 to October 2009, 3 patients with locally recurrent nasopharyngeal carcinoma treated with IMRT combined with Abitol were treated in the first affiliated Hospital of Guangxi Medical University: (1) the 3 patients were all female, aged 38 years, respectively. 53 and 39 years; (2) all of the 3 patients were diagnosed as T3N0M0 stage, 1 case had recurrence after conventional radiotherapy for 7 years, 2 cases had recurrence within 1 year after intensity modulated radiotherapy, 2 cases had recurrence within 1 year after intensity modulated radiotherapy (IMRT). Two patients were treated with 7 field intensity modulation, one patient with radiotherapy dose of GTV58Gy, CTV48Gy, was treated with Cyber knife, one case with radiotherapy dose of GTV68Gy, CTV 58 Gy, the other case with 9 field intensity modulation, GTV70Gy,. CTV 60 Gy; (7) all of the 3 patients were treated with Ebitol molecular targeted therapy. The drug was taken 1 week before radiotherapy and 400 mg / m2 from week 2 to the end of radiotherapy. Results: all the 3 patients were able to tolerate the treatment. The focus of MRI, was obviously improved and the symptoms of cranial nerve were improved in a short time. Conclusion: intensity modulated radiotherapy combined with molecular targeting therapy can improve the local symptoms of patients with locally recurrent nasopharyngeal carcinoma.
