屋尘螨特异性免疫治疗抑制IgE易化变应原结合的检测方法的建立和验证
[Abstract]:Specific immunotherapy for allergic rhinitis has been carried out in many places, and good clinical results have been obtained. Specific immunotherapy can induce a series of immunological changes, including antibody reaction and cell reaction, in which serum specific IgG4 is considered to be the main component of inhibitory blocking antibody induced by specific immunotherapy. However, the detection of the expression level of any single immunological product does not provide a convincing explanation for the clinical improvement brought about by specific immunotherapy. It is urgent to find a biological marker which comprehensively reflects the inhibition of serum antibody induced by specific immunotherapy. IgE facilitated allergen presentation (IgE-facilitated allergen presentation, FAP) involves many links and aspects of the occurrence of allergic reactions, and specific immunotherapy can inhibit this process. The in vitro assay can be reduced to IgE facilitated allergen binding (IgE-facilitated allergen binding, FAB), which can be detected by a simple flow cytometry method. Allergens were incubated with serum containing high concentration of specific IgE to form allergen-IgE complex. After binding to the low affinity receptor of IgE on the surface of B cells transformed by EB virus, the allergen-IgE complex could be detected by flow cytometry. After the addition of specific immunotherapy, the formation of allergen-IgE complex and its subsequent binding to B cells were inhibited, while the percentage of B cells bound to B cells decreased in flow cytometry. That is, after specific immunotherapy, serum therapy can inhibit FAB, and the degree of inhibition is directly related to the degree of clinical improvement caused by specific immunotherapy. The inhibitory activity of serum against FAB after specific immunotherapy became a good biomarker to indicate the clinical effect of specific immunotherapy. In this study, in accordance with the guidelines of International Conference on Harmonisation (ICH) 's experimental methodology, a standardized and clinical test for the detection of IgE facilitation allergens in house dust mite-specific immunotherapy was carried out. It was found that the detection method had small intra-and inter-assay variation, good repeatability, good sensitivity, linearity and robustness, and high clinical sensitivity and specificity. It is a good biological marker to monitor the specific immune serum inhibitory activity, and it has important guiding significance to indicate the effect of specific immune therapy.
