远视儿童应用1%环喷托酯散瞳验光的研究

【摘要】:【目的】探讨远视儿童应用1%环喷托酯滴眼液以及联合应用表面麻醉药丙美卡因滴眼液的远视屈光度和瞳孔散大值的峰值时间,并比较应用1%环喷托酯与1%阿托品散瞳验光的屈光度值的差异。 【方法】采用简单随机化分组方法将71例(141眼)远视儿童(年龄7.9±2.1岁)分为A组(Pro+Cyc组)和B组(NS+Cyc组),所有儿童均进行视力、眼位、眼球运动、裂隙灯及眼底检查。散瞳验光检查步骤如下:(1)A组应用盐酸丙美卡因滴眼液滴眼1次;5分钟后应用1%环喷托酯滴眼3次,每次1滴,间隔10分钟;(2)B组应用生理盐水滴眼1次,5分钟后应用1%环喷托酯滴眼,用法同A组。对A、B两组分别于1%环喷托酯首次滴眼后30、40、50、60、70分钟进行电脑验光和瞳孔直径测量。两组均于1周后给予1%阿托品凝胶滴眼,每天3次,连用3天后重复上述测量,分别为A’组及B’组。采用SPSS16.0软件包对数据进行统计学分析。 【结果】 (1)A组和B组屈光度的峰值分别出现在1%环喷托酯首次滴眼后50分钟和60分钟。 (2)A组远视屈光度的峰值(+4.44±2.34 D)小于A’组屈光度均值(+4.86±2.29D),两者差异有显著的统计学意义(t=11.16,P0.01);B组远视屈光度的峰值(+4.50±2.19 D)小于B’组屈光度均值(+5.04±2.10 D),两者差异亦有显著的统计学意义(t=11.44,P0.01)。A组远视屈光度的峰值与A’组屈光度值的差值(0.42±0.32 D)小于B组远视屈光度的峰值与B’组屈光度值的差值(0.54±0.39D),两差值之间的差异有统计学意义(t=-1.99,P=-0.48)。 (3)A组和B组瞳孔直径的峰值分别出现在1%环喷托酯首次滴眼后50分钟和60分钟,均与其屈光度的峰值时间一致。 (4)A’组与A组屈光度峰值的差值与A’组屈光度值、B’组与B组屈光度峰值的差值与B’组的屈光度值均无相关性;A’组与A组屈光度峰值的差值与年龄无相关性,B’组与B组屈光度峰值的差值与年龄呈负相关。 (5)A'-A50min组、B'-B60min组伴有内斜视及不伴有内斜视两组之间的屈光度差值的均值之间分别进行两两比较,差异均无统计学意义(P0.05)。 [结论]在我国3-14岁远视儿童中,单独应用1%环喷托酯滴眼3次,在首次滴眼后50分钟至70分钟验光可测得其最大的屈光度值;1%环喷托酯联合应用表面麻醉剂丙美卡因可使其峰值时间提前约10分钟,并可增强1%环喷托酯的睫状肌麻痹作用。单独应用1%环喷托酯或者丙美卡因与1%环喷托酯联合应用,均不能完全代替1%阿托品的睫状肌麻痹作用。
[Abstract]:[Objective] To study the peak time of 1% cyclopentol eye drops and the peak time of the eye drops of the combined application of proparacaine eye drops, and to compare the difference of the refractive power of 1% cyclopentol and 1% atropine for the mydriasis. [Methods] 71 (141 eyes) hypermetropia children (aged 7.9 to 2.1 years) were divided into group A (Pro + Cyc group) and group B (NS + Cyc group) by a simple and randomized grouping method. All children were divided into group A (Pro + Cyc group) and group B (NS + Cyc group), and all children underwent visual, ocular, eye movement, slit lamp and fundus examination. Analysis. (3) The peak of the pupil diameter in group A and group B respectively occurred in 50 minutes and 60 minutes after the first drop of the 1% ring-spray, both of which The peak time of the luminosity is consistent.
