应用OCT和GDxVCC检测高度近视眼合并原发性开角型青光眼视网膜神经纤维层厚度的研究

【摘要】:目的 通过应用光学相干断层成像术(Optical coherence tomography, OCT)、偏振激光扫描仪联合个体化角膜补偿技术(Scanning laser polarimetry with variable corneal compensation, GDxVCC)检测高度近视眼(High myopia, HM)合并原发性开角型青光眼(Primary open-angle glaucoma, POAG)的视网膜神经纤维层(Retinal nerve fiber layer, RNFL)的厚度,分析HM合并POAG的视网膜神经纤维层在OCT、GDxVCC检测中的厚度特征,分析OCT、GDxVCC参数相关性,探讨OCT、GDxVCC对高度近视眼合并原发性开角型青光眼的诊断作用。 方法 选取等效球镜(Spherical equivalent, SE)-6.0D--12.0D的高度近视患者12例(12只眼),为HM组;选取SE-6.0D--12.0D高度近视眼合并原发性开角型青光眼的患者12例(12只眼),为HM-POAG组,分别进行GDxVCC检测、OCT快速RNFL厚度扫描(Fast RNFL thickness)、Humphrey电脑自动视野计检查。得到GDxVCC参数:上方RNFL平均厚度(Superior Average)、下方RNFL平均厚度(Inferior Average)、椭圆环内平均RNFL厚度(TSNIT Average)、神经纤维指数(Nerve Fiber Indicator, NFI);OCT参数:上方平均RNFL厚度(Savg)、下方RNFL平均厚度(Iavg)、鼻侧RNFL平均厚度(Navg)、颞侧RNFL平均厚度(Tavg).全周RNFL平均厚度(Avg.Thick);视野参数:平均缺损(Mean defect, MD)、模式标准差(Pattern standard deviation, PSD)。并采用SPSS16.0统计软件包,对所得到的RNFL厚度参数和视野参数进行统计分析。 结果 HM组与HM-POAG组在年龄、SE无统计学差异(P0.05)。与HM组比较,HM-POAG组在参数Superior Average、Inferior Average、TSNIT Average、Savg、 Iavg、Tavg、Avg、Thick数值变小,NFI数值变大,均有统计学差异(P0.05);两组在OCT参数Navg无统计学差异(P0.05);相关性分析TSNIT Average和Avg.Thick相关性好,相关系数r=0.764(P0.05);TSNIT Average、Avg.Thick和MD相关系数分别为0.727、0.637(P0.05);TSNIT Average、Avg.Thick和PSD相关系数别为-0.735、-0.774(P0.05)。GDxVCC.OCT各参数值Superior Average、Inferior Average. TSNIT Average、NFI、Savg、Iavg、Navg、Tavg、Avg.Thick受试者工作曲线下面积(Area Under Receiver Operating Characteristic Curve, AUROC)0.972.0.882.0.958.0.993.0.896.0.927.0.576.0.958.0.965.NFI的AUROC为最大。结论 1HM-POAG组RNFL厚度在Superior Average. Inferior Average. TSNIT Average、Savg、Iavg、Tavg、Avg.Thick均较HM组明显变薄,两组在Navg无显著性差异。 2.OCT和GDxVCC在检测HM组与HM-POAG组RNFL厚度有良好的一致性。 3.OCT及GDxVCC均对HM合并POAG有很好的诊断价值,GDxVCC优于OCT,诊断效能从大到小依次为NFI Superior Average Avg.Thick TSNIT Average、Tavg Iavg SavgInferior Average Navg, Navg诊断效能差。

Methods 12 patients (12 eyes) with high myopia with equivalent spherical (Spherical equivalent, SE)-6.0D--12.0D were selected as HM group, and 12 patients (12 eyes) with SE-6.0D--12.0D high myopia complicated with primary open-angle glaucoma were selected as HM-POAG group for GDxVCC detection and OCT rapid RNFL thickness scan (Fast RNFL thickness), Humphrey computer automatic myopic examination. The GDxVCC parameters were obtained: the RNFL average thickness below the upper RNFL average thickness (Superior Average), the average RNFL thickness in the elliptical ring of (Inferior Average), the (TSNIT Average), nerve fiber index (Nerve Fiber Indicator, NFI); OCT parameter: the upper average RNFL thickness (Savg), the RNFL average thickness (Iavg), nasal RNFL average thickness (Navg), temporal RNFL average thickness (Tavg). Full-week RNFL mean thickness (Avg.Thick); visual field parameters: average defect (Mean defect, MD), model standard deviation (Pattern standard deviation, PSD). The thickness parameters and field parameters of RNFL are statistically analyzed by using SPSS16.0 software package.
