医院信息系统在抗菌药物管理中的应用研究

【摘要】:目的:本课题旨在借鉴国内外临床抗菌药物管理方面的经验,针对国内临床抗菌药物管理方面存在的问题,研究临床抗菌药物管理计算机网络系统应用于临床的效果。开发医院临床抗菌药物用药情况查询和分析软件,利用计算机网络,快速检索和统计全院抗菌药品使用情况。通过分析临床抗菌药物的使用频率以及抗菌药物处方占比、联合用抗菌药频率、抗菌药物使用强度等指数评估医院抗菌药物管理水平。以期达到切实提高医院抗菌药物管理、最大限度地实现“以病人为中心”的医疗服务目的。所进行的具体应用有望使抗菌药物管理水平能够显著提升,提高医院抗菌药物管理水平。 方法:通过调查、问询,随访临床专家,医师及药师等研究抗菌药物使用现状及原因;选取2013年7月至2014年6月期间,在某二级综合医院应用抗菌药物管理系统后为实施组,同时在信息库中抽调出2012年7月至2013年6月期间在该院未应用抗菌药物管理系统前为对照组,两组进行比较;统计分析采用SPSS17.0和Excel2007统计软件进行数据分析。其中计量资料以均数和标准差(X2±S)表示,两组之间的比较采用t检验;计数资料应用χ检验。差异的显著性以P0.05为有统计学意义。 结果:信息化管理是医院今后发展的必然趋势,抗菌药物管理系统(AMIS)的运用,,大大优化了医院业务流程,提升了医院管理水平,建立了以患者为中心的服务模式,实现了良好的社会效益、经济效益、管理效益,达到了医院信息化建设的根本目标。本研究显示,应用抗菌药物管理系统后,实施组的门诊抗菌药物处方数较对照组降低了6.43%,实施组的抗菌药物使用人次占出院人次比例较对照组降低了4.11%,实施组的I类切口手术抗菌药物使用率较对照组降低了4.8%,实施组的住院患者抗菌药物使用强度(DDD值)较对照组降低了28.88,经检验P值均0.05,具有统计学意义,提示两组间有显著差别。 结论:本文就抗菌药物管理系统平台的应用背景和抗菌药物管理系统的价值和方式进行了总结概述,并阐述了通过实践应用系统平台监测抗菌药物管理的主要效果。通过比较分析,验证了该项研究对抗菌药物管理水平的推进和显著提升。临床抗菌药物管理计算机网络系统为临床抗菌药物使用提供了有效的指导和监测手段。抗菌药物管理系统已成为我院管理、临床医药工作必不可少的工具,可为临床抗菌药物管理提供数据分析处理支持,提高临床抗菌药物管理水平,规范医疗行为,提高医疗质量。
[Abstract]:Objective: the purpose of this project is to draw lessons from the experience of clinical antimicrobial management at home and abroad and to solve the problems existing in the management of domestic clinical antimicrobial agents. To study the effect of clinical antimicrobial drug management computer network system applied in clinic, to develop the software for inquiry and analysis of clinical antimicrobial drug use in hospital, and to use computer network, By analyzing the frequency of use of clinical antimicrobials and the proportion of antimicrobial prescriptions, the frequency of combined use of antimicrobial drugs was analyzed. To evaluate the management level of antimicrobial drugs in hospitals with the index of antimicrobial use intensity, so as to improve the management of antimicrobial drugs in hospitals, It is expected that the management level of antimicrobial agents will be improved significantly and the management level of antimicrobial agents in hospitals will be improved. Methods: through investigation, inquiry, follow up of clinical experts, doctors and pharmacists, to study the current situation and reasons of antimicrobial drug use, and to select the application of antimicrobial management system in a level II general hospital between July 2013 and June 2014 as the implementation group. At the same time, from July 2012 to June 2013, the control group was selected as the control group before the use of antimicrobial drug management system in the hospital from July 2012 to June 2013, and the two groups were compared. The statistical analysis was carried out by SPSS17.0 and Excel2007 statistical software. The measurement data were expressed as mean and standard deviation X2 卤S, the comparison between the two groups was t test, and the count data was analyzed by 蠂 ~ 2 test. The significance of the difference was P0.05. Results: information management is the inevitable trend of hospital development in the future. This study shows that, after the application of antimicrobial drug management system, The number of antibiotics prescriptions in the outpatient department of the implementation group was 6.43 lower than that in the control group, and the proportion of antibiotics used in the treatment group was 4.11% lower than that in the control group. The utilization rate of antibiotics in the group I was lower than that in the control group. The DDD value of the inpatients in the implementation group was 28.88 lower than that of the control group, and the P value was 0.05, which was statistically significant. The results suggest that there is a significant difference between the two groups. Conclusion: in this paper, the application background of antimicrobial management system and the value and method of antimicrobial management system are summarized and summarized. The main effect of monitoring antimicrobial drug management through the platform of practical application system is expounded. The computer network system of clinical antimicrobial management provides effective guidance and monitoring means for clinical use of antimicrobial agents. It has become the management of our hospital. The essential tools of clinical medicine work can provide data analysis and processing support for clinical antimicrobial drug management, improve the management level of clinical antimicrobial agents, standardize medical behavior and improve medical quality.
