安定对新生大鼠缺血缺氧性脑损伤保护作用及其机制的研究

【摘要】:新生儿缺血缺氧性脑损伤(hypoxia-ischaemia brain damage, HIBD)是指围产期新生儿窒息引起的脑部综合征,常可引起不可逆的脑组织损伤和一系列神经系统后遗症,是新生儿病死及发生神经系统残疾的主要原因之一。目的:在本研究中,探讨安定对缺血缺氧性脑损伤新生大鼠的近远期脑保护作用,并对安定可能的脑保护作用新机制进行了初步探讨,为临床治疗HIBD提供参考。方法:7日龄新生Sprague-Dawley (SD)大鼠,随机分为三组(每组20只):假手术组(Sham)、缺血缺氧模型组(HIBD)和安定治疗组(Diazepam)。HIBD模型组参照经典Rice-Vannucci法制作HIBD模型;假手术组仅游离出左侧颈总动脉,不做缺血缺氧处理。模型制备成功后安定治疗组立即腹腔注射剂量为10mg.kg-1.8h-1的安定,连续给药3天;其他两组给予相同剂量的生理盐水。通过氯化2,3,5-三苯基四氮唑(2,3,5-triphenyltetrazolium chloride, TTC)染色以及自主活动和Morris水迷宫实验来探讨安定对新生HIBD大鼠的近远脑期保护作用。同时,通过Real-time PCR、Western-blot以及免疫组织化学实验来检测KCC2mRNA、KCC2蛋白水平的表达情况。 结果: (1)新生大鼠HIBD模型的鉴定:新生大鼠缺血缺氧完成后,动物出现明显的左旋症状,说明HIBD模型建立成功。 (2) TTC染色结果显示:HIBD模型组有大面积的白色脑梗死区域,再次说明HIBD模型的制备是成功的。与HIBD模型组相比,安定治疗组的脑梗死区域明显减少(P0.01),提示安定对于HIBD新生大鼠有一定的近期脑保护作用。 (3)自主活动实验结果显示:和假手术组相比,HIBD模型组的活动明显减少(P0.05),给予安定治疗后活动有所增加,但无显著差异(P0.05)。 (4) Morris水迷宫实验结果显示:在定位航行试验中,5天总平均逃避潜伏期为HIBD组>安定治疗组>假手术组,从第三天开始各组之间相比结果有差异(P0.05or P0.01);在空间探索试验中,HIBD模型组大鼠的目标象限停留时间百分比和穿越站台次数明显减少,给予安定治疗后有一定的改善(P0.05orP0.01)。说明HIBD后大鼠学习记忆能力下降,安定治疗能够减轻HIBD对远期学习记忆能力的损害。 (5) RT-PCR、Western-blot以及免疫组织化学实验结果显示:KCC2-mRAN和蛋白表达水平在HIBD后显著下调(P0.05or P0.01),给予安定治疗后有一定的提高。 结论: (1)早期给予安定治疗可以明显减少HIBD新生大鼠的脑梗死体积,表明安定对HIBD损伤有一定的早期保护作用; (2)早期给予安定治疗可以明显改善HIBD新生大鼠的自主活动能力和空间学习记忆能力,表明安定可以改善远期神经系统预后; (3)安定通过阻止KCC2在mRAN和蛋白水平上表达的下降,从而发挥脑保护作用。 综上所述:安定对新生大鼠缺血缺氧性脑损伤有一定的近远期保护作用,,其可能的作用机制是通过防止KCC2表达的下调,维持细胞内Cl-的稳态,确保GABA发挥抑制性神经递质的作用,从而起到脑保护的作用。
