HACCP方法在我院药品调剂差错风险管理中的应用

【摘要】:目的:加强我院药品调剂差错的全面管控,保障患者用药安全。方法:介绍我院根据危害分析和关键控制点法(HACCP)实施药品调剂差错风险管理的步骤和相关内容,并通过比较实施前(2015年)、后(2016年)的调剂差错率评价管理效果。结果:我院从建立工作组入手,绘制药品调剂流程图,根据HACCP原则制订计划[包括危害分析,确定关键控制点(CCP),建立关键限值(CL)、监测体系、纠偏措施、确认程序和质量记录档案],确定4个CCP即审核处方、调配药品、书写药袋或粘贴标签、向患者交付药品。针对这些CCP制订并实施了相应的措施,实现了对药品调剂的有效管控。与实施HACCP前比较,实施HACCP后我院药品调剂差错率下降84.73%(0.753‰vs.0.115‰)。结论:将HACCP科学地运用于医院药品调剂环节的管理,可以有效控制药品差错风险,促进患者的用药安全。
[Abstract]:Objective: to strengthen the comprehensive control of drug dispensing errors in our hospital and ensure the safety of drug use. Methods: according to hazard analysis and critical control point method (HACCP), this paper introduces the steps and related contents of error risk management of drug dispensing, and evaluates the management effect by comparing the adjustment error rate before (2015) and after (2016). Results: starting with the establishment of the working group, our hospital drew up the flow chart of drug dispensing, worked out the plan according to the HACCP principle [including hazard analysis, determined the critical control point (CCP),)] to establish the critical limit value (CL),) monitoring system, and corrected the deviation. Confirm the procedure and quality record file], determine 4 CCP that is to review the prescription, mix the medicine, write the medicine bag or affix the label, and deliver the medicine to the patient. In view of these CCP, the corresponding measures are worked out and implemented, and the effective control of drug dispensing is realized.
