北京等六地区三级医院抗菌药物临床应用专项整治活动效果分析

【摘要】：目的:为合理使用抗菌药物提供参考。方法:从北京、天津、上海、杭州、广州、成都六地区三级医院中选择69家作为样本医院,抽取其2009-2013年抗菌药物的采购与门诊使用数据,从抗菌药物占全部化学药采购金额比例、化学药销售金额前80%药品中抗菌药物的个数和品种数、门诊处方抗菌药物使用率、门诊抗菌药物处方中两种及以上抗菌药物联用百分率以及各种类抗菌药物使用比例等方面进行分析。结果:平均抗菌药物占化学药采购金额比例从2009年的20.97%下降至2013年的14.17%,化学药销售金额前80%药品中抗菌药物的个数从2009年的43个下降至2013年的34个,门诊处方抗菌药物平均使用率从2009年的23.22%下降至2013年的16.11%,以上指标比较差异均有统计学意义(P0.05)。平均抗菌药物的品种数从2009年的78种下降至2012年的54种(P0.05),但是2013年又回升至66种;门诊抗菌药物处方中两种及以上抗菌药物联用百分率从2009年的9.02%下降至2012年的7.35%(P0.05),2013又回升至12.89%;门诊抗菌药物处方中抗菌药物注射剂使用率各年度没有明显的变化;同时分别对六地区三级医院各种类抗菌药物的使用情况进行分析,头孢菌素类药物一直排在第1位,几乎是总抗菌药物使用的一半,但比例有下降趋势,而特殊使用级抗菌药物使用率则逐年上升。结论:经过3年抗菌药物专项整治,北京等六地区抗菌药物使用情况整体上有了明显改进,但个别指标改善不大,某些指标2013年有反弹,因此此项工作需要建立持续督导长效机制。
[Abstract]:Objective: to provide reference for rational use of antimicrobial agents. Methods: 69 tertiary hospitals in Beijing, Tianjin, Shanghai, Hangzhou, Guangzhou and Chengdu were selected as sample hospitals. The data of antimicrobial procurement and outpatient use from 2009 to 2013 were collected. According to the proportion of antimicrobial drugs to the total purchase amount of chemical drugs, the number and variety of antimicrobial drugs in the 80% of the total amount of chemical drugs sold, and the utilization rate of antibiotics in out-patient prescriptions, The percentages of two or more antimicrobial agents and the proportion of antimicrobial agents used in outpatient prescriptions were analyzed. Results: the average proportion of antimicrobial drugs in the purchase of chemical drugs decreased from 20.97% in 2009 to 14.17% in 2013, and the number of antibiotics in the 80% of the total amount of money sold decreased from 43 in 2009 to 34 in 2013. The average utilization rate of antibiotics in outpatient prescriptions decreased from 23.22% in 2009 to 16.11% in 2013, and the differences were statistically significant (P0.05). The average number of antimicrobial drugs decreased from 78 in 2009 to 54 in 2012 (P0.05), but rose to 66 in 2013. The percentage of combined use of two or more antimicrobial drugs in outpatient antimicrobial prescriptions decreased from 9.02% in 2009 to 7.35% in 2012 (P0.05) and rose again to 12.89% in 2013, and there was no significant change in the utilization rate of antimicrobial injections in out-patient antimicrobial prescriptions in each year. At the same time, the use of various kinds of antimicrobial drugs in tertiary hospitals in the six regions was analyzed respectively. Cephalosporins have been ranked first, almost half of the total use of antimicrobial drugs, but the proportion has a downward trend. The rate of use of special-use class antibiotics increased year by year. Conclusion: after three years of special treatment of antimicrobial agents, the use of antimicrobial drugs in Beijing and other six regions has improved obviously, but the improvement of individual indicators is not significant, and some indicators have rebounded in 2013. Therefore this work needs to establish the continuous supervision long-term effect mechanism.
