利福平药动学影响因素的研究进展

【摘要】：目的:为临床合理应用利福平(RFP)以及实现其个体化用药提供参考。方法:查阅Pub Med、中国知网等数据库中收录的(1985-2015年)RFP的药动学及相关影响因素研究文献,并据此进行归纳和总结。结果:患者合并疾病、药物剂型、联合用药及食物等因素对于RFP吸收方面存在影响;患者合并疾病、性别、体质量等因素对于RFP分布方面存在影响;患者生活习惯、性别、年龄、体质量、基因多态性、联合用药及结核杆菌基因变异等因素对于RFP代谢方面存在影响。结论:RFP的药动学受到多种因素影响,在临床结核病治疗工作中,除常规的RFP血药浓度监测外,还应开展相关基因多态性检测以便调整剂量和防止耐药的发生,并综合考虑患者多方面情况包括基因型来制订个体化给药方案,以促进临床合理用药。
[Abstract]:Objective: to provide reference for the rational application of rifampicin (RFP) and the realization of individualized drug use. Methods: the literature on pharmacokinetics and related influencing factors of RFP (1985-2015) included in Pub Med, database were reviewed and summarized. Results: the factors such as patients complicated with disease, drug dosage form, combined use of drugs and food had an effect on the absorption of RFP, and the factors of patients complicated with disease, sex, body mass and other factors had an effect on the distribution of RFP. Life habits, sex, age, body mass, gene polymorphism, combined use of drugs and gene variation of Mycobacterium tuberculosis had effects on RFP metabolism. Conclusion: the pharmacokinetics of RFP is affected by many factors. In addition to the routine monitoring of serum drug concentration of RFP, we should also carry out the detection of gene polymorphism in order to adjust the dosage and prevent the occurrence of drug resistance in the treatment of tuberculosis. In order to promote rational use of drugs in clinic, individualized drug administration was made by taking into account various conditions of patients, including genotypes.
