热浪对心血管疾病影响的机理研究

【摘要】:在全球气候变暖的大背景下,南京市夏季高温热浪过程频发,心血管疾病发病和死亡率均有明显增加。在热浪影响心血管疾病研究方面,大多数研究者多采用流行病学统计方法,而针对热浪过程诱发和加重心血管疾病的相关机制方面仍缺乏深入探讨,且对热浪的研究多局限于持续高温的热浪过程,对期间温度骤降影响的研究较少涉及。因此,本项研究通过对南京市历史气象资料的统计分析,结合人群死亡资料,得到热浪过程对心血管疾病的影响特征;在此基础上,通过一次典型热浪过程对动物心血管疾病影响的实验,探讨热浪过程诱发和导致患者死亡的机理。主要研究结论如下:(1)2005—2008年南京市夏季共发生8次热浪过程,其中,持续高温的热浪过程及有降温的热浪过程各发生4次。死于心脑血管疾病的人数占总死亡人数的30%-40%。而热浪过程期间死于心脑血管疾病的65岁及以上的老年人比例达73.96%。有强降温发生的热浪过程造成的死亡人数明显多于持续高温的热浪过程。(2)小鼠受到热刺激时,HSP60及TNF-α、sICAM-1等炎性细胞因子显著升高,使得体内炎症反应加强,SOD活性降低血脂升高,导致血管硬化发生发展,这可能是热浪过程增加心血管病风险的一个诱因。(3)热刺激会诱使机体炎症标志物水平显著升高,可能会加重动脉粥样硬化疾病的发生发展,温度骤降过程导致血管迅速收缩,将极大的促进动脉粥样硬化的加重和血栓的形成,可能是导致最终心血管病发病和死亡的直接原因。
[Abstract]:In the background of global warming, Nanjing City summer heatwave process frequently, cardiovascular disease morbidity and mortality were significantly increased. In the heat effect of cardiovascular disease research, most researchers use epidemiological statistical methods, and according to the aspects of process and related mechanism of heat induced exacerbation of cardiovascular disease is the lack of in-depth discussion and research on the heat is limited to continuous high temperature heat process, study on the influence of the period of the temperature drop of the less involved. Therefore, this study based on the statistical analysis of historical meteorological data of Nanjing City, with the population of death data are characteristic of heat wave impact on cardiovascular disease; on this basis, through the influence of a typical process of heat cardiovascular disease animal experiments, to explore the process and mechanism of heat induced cause of death in patients. The main conclusions are as follows: (1) From 2005 to 2008 in Nanjing city in summer were 8 heatwave, the continuous high temperature heat process and cooling process of each heat occurred 4 times. Deaths from cardiovascular and cerebrovascular diseases accounted for the total number of deaths and the proportion of heat during the process of 30%-40%. died of cardiovascular disease at the age of 65 and the elderly have reached 73.96%. the number of deaths occurred in the process of strong cooling heat caused significantly more than the continuous high temperature heat process. (2) mice by thermally stimulated, HSP60 and TNF- alpha, sICAM-1 and other inflammatory cytokines increased significantly, the inflammatory reaction in vivo enhanced SOD activity decreased serum lipid levels, resulting in the development of vascular sclerosis, this may be a the heat process causes increased risk of cardiovascular disease. (3) will induce inflammatory markers significantly increased levels of heat stimulation may aggravate the occurrence and development of atherosclerotic disease, the temperature drop process The rapid contraction of blood vessels will greatly promote the aggravation of atherosclerosis and the formation of thrombosis, which may be the direct cause of the final occurrence and death of cardiovascular diseases.
