我国公立医院改革的路径与对策研究

【摘要】:医院作为保障人民生活健康的服务行业,,它的发展状况在相当程度上反映了一个国家的国民生活质量和社会福利水平。随着我国社会经济的迅速发展以及人们对健康需求的逐步增强,“看病就医”问题已经成为关系人民群众日常生活最切身的根本利益之一。在我国,公立医院所占比例为95%左右,是老百姓最主要的就医场所,由于目前我国的公立医院运行机制和管理体制比较滞后,导致近年来老百姓“看病难、看病贵”问题愈发凸显,这一矛盾的激化也最终诱发了我国新一轮医改的实施。因此,研究公立医院改革问题具有一定的理论价值和实践意义。 本文以新公共管理理论、政府治理理论和公平与效率关系理论作为理论基础,对现阶段我国公立医院存在的问题进行了详细分析,同时结合国内现行案例分析和国外公立医院改革的经验借鉴,最后提出对策建议。本文在结构上分为7章。其中:第1章为导论;第2章相关理论概述;第3章我国公立医院运行过程中的突出矛盾;第4章为我国公立医院改革的地位和目标;第5章为我国公立医院改革的路径分析;第6章为深化公立医院改革的对策建议;第7章为结论与展望。 文章重点分析了目前我国公立医院在经营、发展和运行中存在的突出矛盾,然后从公立医院改革的价值取向、新医改中的公立医院改革以及公立医院改革中的管理体制改革和我国公立医院改革法人治理结构的目标模式等四个方面详细论述了我国公立医院改革的目标以及未来的发展方向,并对我国公立医院新一轮改革的第一批试点城市的试点情况进行了介绍,同时对各地在探索公立医院改革中的经验进行了分享,对存在不足进行了评价,然后挑选个别公立医院改革案例进行论证分析,列举了几个在公立医院改革方面取得成效的国家的经验借鉴,最后从优化整体医疗资源配置、改革公立医院补偿机制、改革公立医院管理体制、改革公立医院治理机制、改革公立医院医务人员激励机制等方面对我国公立医院改革提出具体对策建议。
[Abstract]:As a service industry to guarantee people's health, the development of hospital reflects to a certain extent the quality of life and the level of social welfare of a country. With the rapid development of our country's social economy and the gradual enhancement of people's health demand, the problem of "seeing a doctor and seeking medical treatment" has become one of the most important fundamental interests of people's daily life. In our country, the proportion of public hospitals is about 95%, which is the most important place for ordinary people to seek medical treatment. Because of the lagging operation mechanism and management system of public hospitals in our country, in recent years, it is difficult for ordinary people to see a doctor. The problem of expensive medical treatment becomes more and more prominent, and the intensification of this contradiction finally induces the implementation of a new round of medical reform in China. Therefore, the study of public hospital reform has certain theoretical value and practical significance. Based on the theory of new public management, the theory of government governance and the theory of the relationship between fairness and efficiency, this paper makes a detailed analysis of the problems existing in public hospitals in China at this stage. At the same time, combined with the domestic current case analysis and foreign public hospital reform experience, finally put forward countermeasures and suggestions. The structure of this paper is divided into seven chapters. The first chapter is the introduction; the second chapter is the summary of relevant theories; the third chapter is the prominent contradiction in the process of public hospital operation; the fourth chapter is the status and goal of our country's public hospital reform; the fifth chapter is the path analysis of our country's public hospital reform; Chapter 6 is the countermeasures and suggestions for deepening the reform of public hospitals, and chapter 7 is the conclusion and prospect. This paper analyzes the prominent contradictions in the management, development and operation of public hospitals in China, and then analyzes the value orientation of the reform of public hospitals. In this paper, the public hospital reform in the new medical reform, the management system reform in the public hospital reform and the target mode of the legal person governance structure of the public hospital reform in our country are discussed in detail, and the goal of the public hospital reform in our country is discussed in detail. The future direction of development, It also introduces the first batch of pilot cities in the new round of reform of public hospitals in our country, shares the experiences of other places in exploring the reform of public hospitals, and evaluates the shortcomings. Then select individual cases of public hospital reform for demonstration and analysis, enumerate the experience of several countries that have achieved good results in the reform of public hospitals, and finally optimize the overall allocation of medical resources and reform the compensation mechanism of public hospitals. To reform the management system of public hospitals, to reform the governance mechanism of public hospitals, and to reform the incentive mechanism of medical personnel in public hospitals, this paper puts forward concrete countermeasures and suggestions for the reform of public hospitals in our country.
