基于成熟度模型的医疗设备管理能力评估体系的构建

【摘要】:目的:本研究借鉴JCI国际标准、ISO国际标准和等级医院评审标准的思想,以ISO9001—2015版本作为模板构建医疗设备管理能力评价指标体系,并确定各指标权重。在此基础上,借鉴CMM能力成熟度的思想,运用ISO9001质量管理成熟度通用模型进行医疗设备管理能力成熟度评价,解决等级医院评审和JCI评审等达标性评审的不足和局限性,分析医院设备管理部门设备管理水平,同时为医院内部评审和政府等第三方组织评价提供一定的参考依据。方法:查阅文献,了解医疗设备管理现状及管理能力评估办法,初步构建医疗设备管理能力评估指标体系;通过两轮Delphi修改和完善医疗设备管理能力评估指标体系;运用AHP计算各指标权重并赋予相应分值;根据ISO9001质量管理成熟度通用模型及评审指南,对每个指标打分,并计算各指标得分之和,即成熟度评价。利用Excel2003、SPSS l3.0软件进行数据处理分析。结果:1.医疗设备管理能力评估指标体系的构建通过两轮专家咨询对指标条目进行修改与筛选,最终构建医疗设备管理能力评估指标体系,包括7个一级指标:背景及环境、领导作用、核心管理流程及程序、人员能力、设备文件管理、绩效评价及改进、质量安全文化,25个二级指标。2.德尔菲评价指标结果两轮专家咨询结果显示,专家的积极系数为1、0.96,专家积极性较高;熟悉程度系数、判断系数和权威系数分别为0.71、0.92、0.81,专家咨询权威程度较高。两轮专家咨询的协调系数分别为0.234、0.245,专家意见协调性较好,对指标认可度较高。3.医疗设备管理能力评估指标体系权重的设置通过AHP得出医疗设备管理能力评估指标体系各条目的权重得分,其中一级指标所的权重为背景及环境(0.0585)、领导作用(0.2285)、核心管理流程及程序(0.2786)、人员能力(0.1630)、设备文件管理(0.0455)、绩效评价及改进(0.1337)、质量安全文化(0.0922)。4.医疗设备管理能力评估指标体系的检验指标体系信度和效度检验结果显示,总指标体系的分半信度为0.787,克朗巴赫系数为0.879,各一级指标与总指标体系的相关系数在0.627-0.819之间,指标体系的内容具有较好的信度和效度。5.医疗设备管理能力评估指标体系的成熟度评价及实例分析在构建指标体系及设置权重的基础上,提出对医疗设备管理能力进行成熟度评价,采用中国质量协会发布的ISO9001质量管理成熟度评价准则中的成熟度评价通用模型及基本评价方法,结合具体数据进行了实例分析。结果显示,天津市A医院设备管理能力成熟度总体得分为740.97,该院设备管理能力处于第四等级,即管理级。结论:构建了医疗设备管理能力评估指标体系,并确了定指标权重,运用成熟度评价方法,进行医疗设备管理能力成熟度评估的实例研究。指标体系构建的专家咨询结果可靠,信度和效度较好,能够为我国医院医疗设备管理能力的评估及评价提供参考依据。
[Abstract]:Objective: based on the ideas of JCI international standard, ISO international standard and grade hospital evaluation standard, this study uses ISO9001-2015 version as template to construct the evaluation index system of medical equipment management ability, and to determine the weight of each index. On this basis, drawing lessons from the idea of CMM capability maturity, the general model of ISO9001 quality management maturity is used to evaluate the capability maturity of medical equipment management, to solve the shortcomings and limitations of standard evaluation such as grade hospital evaluation and JCI evaluation, to analyze the equipment management level of hospital equipment management department, and to provide some reference for hospital internal evaluation and government and other third-party organization evaluation. Methods: consult the literature, understand the present situation of medical equipment management and the evaluation method of management ability, initially construct the evaluation index system of medical equipment management ability, modify and perfect the evaluation index system of medical equipment management ability through two rounds of Delphi, calculate the weight of each index and give the corresponding score through two rounds of AHP. According to the general model and evaluation guide of ISO9001 quality management maturity, each index is scored and the sum of each index score is calculated, that is, maturity evaluation. The data processing and analysis are carried out by using Excel2003,SPSS l3.0 software. Result: 1. The construction of medical equipment management ability evaluation index system through two rounds of expert consultation to modify and screen the index items, and finally construct the medical equipment management ability evaluation index system, including seven first-level indicators: background and environment, leadership, core management processes and procedures, personnel capacity, equipment file management, performance evaluation and improvement, quality and safety culture, 25 secondary indicators. 2. The results of two rounds of expert consultation showed that the positive coefficient of experts was 1, 0.96, and the enthusiasm of experts was higher, and the familiarity coefficient, judgment coefficient and authority coefficient were 0.71, 0.92 and 0.81, respectively. The coordination coefficients of the two rounds of expert consultation are 0.234 and 0.245. the coordination of expert opinion is better and the degree of approval of the index is higher.
