质谱分析在发现胃早癌标志物的应用

背景胃癌在目前所有恶性肿瘤中发病率位列第五,死亡率位列第三。我国的胃癌发病率和死亡率均处于世界较高水平,每年新发病例约40万例,占世界总发病例数的42%。早期发现、早期诊断和早期治疗是有效改善胃癌预后的方法。消化内镜的诊断效果较好,但作为有创操作,繁琐耗时,不适合大规模人群筛查。近年来,对胃癌等多种肿瘤组织中的磷脂及其相关物质进行研究,发现了磷脂与肿瘤的发生发展间的一些关系。作为潜在的有效肿瘤标志物,磷脂目前在胃癌中的研究还比较缺乏。方法采用MALDI-FTICRMS技术,对实验组5组(对照组、萎缩性胃炎或肠上皮化生组、低级别上皮内瘤变组、高级别上皮内瘤变组、早期胃癌组每组22例,共110例)血清标本进行分析并鉴定磷脂分子。通过统计学方法寻找差异磷脂分子,并进行受试者操作特性曲线(ROC)分析。在验证组中(对照组、早期胃癌组每组24例,共48例)验证实验组差异磷脂分子及其诊断效能。除此之外,对验证组中早期胃癌组患者术前、术后血清标本中的磷脂分子进行鉴定分析,寻找比较术前、术后磷脂分子变化。结果本研究在所有血清中共鉴定19个磷脂分子。实验组中,8个磷脂分子在不同组间存在差异,包括 PA(...
