【摘要】:目的分析云南省2009-2014年登革热流行特征,为登革热防控措施提供依据。方法收集云南省2009-2014年法定传染病报告系统中的登革热数据资料,采用描述性流行病学方法统计分析;采用RT-PCR检测登革病毒血清型和采用蚊虫幼虫采集法调查登革热媒介埃及伊蚊的分布。结果 2009-2014年云南省共报告登革热病例2 040例,其中本地感染病例1 579例,输入病例461例;在输入病例中,缅甸和老挝输入病例比例最高(占79.83%);全年均有病例报告,8~11月为高发季节;病例主要集中于云南边境地区的西双版纳州(1 382例,占67.75%)和德宏州(562例,占27.55%);男女比例为1.05∶1,各年龄组均有发病,以青壮年为主,20~59岁病例1 538例(占75.39%),职业以商业服务、家务及待业、农民和学生为主,分别为523例(占25.64%)、224例(占10.98%)214例(占10.49)和200例(占9.80%)。对景洪市307份血清用RT-PCR和NS1抗原进行检测,两种方法吻合率在70%以上,NS1抗原检测方法敏感度为86.39%,特异度为57.76%;对瑞丽市73份血清用一步法实时荧光RT-PCR检测进行复核,NS1抗原检测方法敏感度为100%,特异度为91.67%;4个登革病毒血清型在云南均有发现,其中西双版纳州景洪市以登革病毒血清型Ⅲ型为主(99.50%),德宏州瑞丽市以Ⅰ型为主(72.00%);白纹伊蚊广泛分布,埃及伊蚊分布区扩大到9个边境县市。结论云南省登革热流行势态较为严峻,周边缅甸和老挝输入病例数逐年增加,媒介埃及伊蚊分布区域不断扩大,建议进一步加强云南边境地区登革热监测和媒介控制工作。
[Abstract]:Objective to analyze the epidemic characteristics of dengue fever in Yunnan Province from 2009 to 2014. Methods Dengue fever data were collected from the legal infectious disease reporting system in Yunnan Province from 2009 to 2014, and descriptive epidemiological methods were used to analyze the data. The distribution of dengue vector Aedes aegypti was investigated by RT-PCR detection of dengue virus serotype and mosquito larva collection. Results A total of dengue fever cases 2 were reported in Yunnan Province from 2009 to 2014. 040 cases. There were 1 579 cases of local infection and 461 cases of imported cases. Myanmar and Laos had the highest proportion of imported cases (79.83%). Cases were reported from 8 to November as the high incidence season in the whole year. There were 1 382 cases in Xishuangbanna prefecture (67.75%) and 562 cases in Dehong Prefecture (27.5555%). The ratio of male to female was 1.05: 1. All age groups had the disease. There were 1 538 cases (75.399%) of male and female aged 2059 years old. The occupation was business service, housework and unemployment. 523 cases (25.64%) were farmers and students respectively. 224 cases (10.98%) and 200 cases (9.80%) were tested for RT-PCR and NS1 antigens in 307 serum samples in Jinghong city. The sensitivity and specificity of the two methods were 86.39 and 57.76, respectively. The sensitivity and specificity of the method for detecting NS1 antigen in 73 sera from Ruili city were 100 and 91.67, respectively.
