胸腔导管留置联合注射药物治疗结核性胸膜炎的效果分析

【摘要】:目的研究胸腔导管留置联合注射地塞米松、异烟肼治疗结核性胸膜炎的应用价值。方法选择2014年1月—2015年12月河北省廊坊市人民医院收治的120例结核性胸膜炎患者,按照随机数字表法将其分成研究组与对照组,每组60例,研究组实施胸腔导管留置联合注射地塞米松、异烟肼治疗;对照组经B超定位胸腔穿刺抽液治疗,比较分析两组临床疗效及不良反应。结果研究组治疗总有效率为95.0%,对照组的治疗总有效率为80.0%,两组治疗总有效率比较差异有统计学意义(P0.05)。研究组患者胸腔积液引流量为(1 103.8±276.6)m L,低于对照组胸腔积液引流量(1 763.2±234.5)ml(P0.05);研究组胸水消失时间为(7.3±2.6)d,短于对照组胸水消失时间(12.4±3.2)d(P0.01);研究组胸膜肥厚粘连发生率为5.0%,低于对照组胸膜肥厚粘连发生率16.7%(P0.05);研究组气胸等不良反应发生率为1.7%,低于对照组气胸等不良反应发生率10.0%(P0.05)。结论胸腔导管留置联合注射地塞米松、异烟肼治疗结核性胸膜炎效果显著,引流胸液量大,胸腔穿刺数较少,胸水消退时间短、不良反应少,值得应用及推广。
[Abstract]:Objective to study the value of thoracic catheter indwelling combined with dexamethasone injection and isoniazid in the treatment of tuberculous pleurisy. Methods 120 cases of tuberculous pleurisy treated in Langfang people's Hospital of Hebei Province from January 2014 to December 2015 were selected. According to the method of random digital table, it was divided into study group and control group with 60 cases in each group. The study group was treated with thoracic catheter indwelling combined with dexamethasone and isoniazid injection. Results the total effective rate of the study group was 95.0, the total effective rate of the control group was 80.00.There was significant difference in the total effective rate between the two groups. The volume of pleural effusion in the study group was 1.103.8 卤276.6mL, which was lower than that in the control group (1763.2 卤234.5ml / ml), the time of pleural effusion disappearance in the study group was 7.3 卤2.6d, which was shorter than that in the control group (12.4 卤3.2dg / ml), and the incidence of pleural hypertrophy in the study group was 5.0, which was lower than that in the control group. The incidence of adverse reactions such as pneumothorax in the study group was 1.7, which was lower than that in the control group (10.0 and P 0.05). Conclusion the incidence of thoracic catheter indwelling combined with dexamethasone injection is lower than that of the control group. Isoniazid is effective in treating tuberculous pleurisy with large amount of draining pleural effusion, less number of pleural puncture, short time of pleural effusion regression and less adverse reactions. It is worth applying and popularizing.
