新疆和布克赛尔蒙古族自治县棘球绦虫感染及人群细粒棘球蚴病危险因素分析

【摘要】:目的:了解新疆和布克赛尔蒙古自治县棘球绦虫感染流行现状与人群细粒棘球蚴病危险因素,为卫生行政部门包虫病的防治提供科学依据。方法:对和布克赛尔蒙古自治县六个乡(牧场)的居民进行整群抽样,抽取1857人并进行问卷调查、B超腹部扫描及血清学抗体检查并对细粒棘球蚴病危险因素进行分析。剖检38只犬,粪抗原ELISA法检测入户收集的126只犬的粪样本,粪抗原PCR法检测其中阳性标本。B超筛查绵羊180只。结果:人群棘球蚴病患病率6.40%(119/1857),其中细粒和泡型棘球蚴病患病率分别为6.19%(115/1857)和0.21%(4/1857),血清阳性率为11.79%(219/1857)。多因素Logistic回归分析,性别、年龄及民族可能是血清阳性危险因素。剖检犬的感染率52.63%(20/38)。犬粪抗原阳性率为39.68%(50/126),粪抗原PCR法检测50份阳性标本,阳性结果有26份。126只犬总体平均驱虫率只有49.20%(62/126)。羊的感染率为15.55%(28/180)。结论:和布克赛尔蒙古自治县人群细粒棘球蚴病患病率和血清阳性率,犬的感染率,羊的感染率均处于较高水平;当地的防治工作仍然任重而道远。
[Abstract]:Objective: to investigate the prevalence of Echinococcus granulosus infection and the risk factors of Echinococcus granulosus in Xinjiang and Bukeser Mongolian Autonomous County. To provide scientific basis for the prevention and control of hydatidosis in health administration department. Methods: the residents of six townships (pastures) in Hebukeser Mongol Autonomous County were selected by cluster sampling. 1857 patients were selected and investigated by questionnaire, abdominal ultrasonography and serological antibody were examined and the risk factors of Echinococcus granulosus were analyzed. The fecal samples of 126 dogs were detected by fecal antigen ELISA method, 38 dogs were dissected. Results: the prevalence rate of echinococcosis was 6.40% / 1857%, in which the incidence of fine grain and alveolar echinococcosis was 6.19115 / 1857) and 0.21 / 41857%, respectively. The positive rate of serum was 11.7921919 / 1857%, multiple factor Logistic regression analysis, sex, sex, gender, and the positive rate of serum was 11.792191.18570.Results: the incidence of echinococcosis was 6.40% / 1857%, and the prevalence of fine-grained echinococcosis and alveolar echinococcosis was 6.19% / 1857%, respectively. Age and nationality may be the risk factors of seropositivity. The infection rate of dog was 52.63% or 20% 38%. The positive rate of dog fecal antigen was 39.68% 50 / 126%. The fecal antigen PCR method was used to detect 50 positive specimens. The average deworming rate of 26 dogs was only 49.20 / 1260.The infection rate of sheep was 15.55 / 28 / 1800.Conclusion: the prevalence of echinococcosis and serum positive rate, the infection rate of dogs and sheep are all in a high level in Boxer Mongolian Autonomous County. There is still a long way to go in the area of prevention and cure.
