HAART治疗前后HIVAN患者血清胱抑素C与糖类抗原-125水平变化及意义

【摘要】：目的观察HIVAN高效抗反转录病毒治疗(HAART)前后人类免疫缺陷病毒相关性肾病(HIVAN)患者血清胱抑素C(Cystatin C)、糖类抗原-125(CA-125)水平变化并探讨其意义。方法选取50例HIVAN患者(观察组),应用罗氏全自动PPE生化仪,分别采用比浊法及电化学发光法检测HAART治疗前及治疗后6个月血清Cystatin C、CA-125水平,并与同期健康体检者50例(对照组)对比。采用COULTER EPICS XL型流式细胞仪测定CD4+T细胞数、CD4+T/CD8+T值。相关性分析采用Spearman相关分析法。结果观察组治疗前血清Cystatin C、CA-125水平明显高于对照组(P均0.01);HAART治疗6个月后,观察组血清CA-125水平下降较明显,与治疗前比较(P0.05),而Cystatin C下降不明显,与治疗前比较差异无统计学意义(P0.05);CD4+T细胞数、CD4+T/CD8+T值升高明显,与治疗前比较差异有统计学意义(P均0.05)。相关分析结果提示,CD4+T细胞与hs-CRP、CA-125呈负相关(r分别为-3.326、-2.910,P均0.05);而CD4+T细胞与Cystatin C、Ccr、GFR无相关性(r分别为0.285、0.101、0.225,P均0.05)。GFR与Cystatin C、CA-125、hs-CRP呈负相关(r分别为-2.352、-4.022、-3.022,P均0.05)。结论 HAART治疗前HIVAN患者血清Cystatin C、CA-125水平明显升高,治疗后血清CA-125水平下降明显;监测血清Cystatin C、CA-125水平,有助于观察和评估HIVAN患者肾脏损害发生、进展和预后。
[Abstract]:Objective to observe the changes of serum levels of cystatin (C(Cystatin), carbohydrate antigen (-125) and carbohydrate antigen (CA-125) in patients with human immunodeficiency virus associated nephropathy (HIV HIVAN) before and after treatment of HAART with HIVAN and to explore its significance. Methods 50 patients with HIVAN were enrolled in the study group. The serum levels of Cystatin CA-125 were detected by turbidimetry and electrochemiluminescence before and 6 months after HAART treatment by using Roche automatic PPE biochemical instrument. The results were compared with 50 healthy persons (control group) in the same period. The number of CD4 T cells and CD4 T/CD8 T were measured by COULTER EPICS XL flow cytometry. Correlation analysis was performed by Spearman correlation analysis. Results the levels of serum Cystatin Con CA-125 in the observation group were significantly higher than those in the control group (P < 0.01). The serum CA-125 level in the observation group was significantly lower than that in the control group (P < 0.05), but Cystatin C was not significantly decreased in the observation group. There was no significant difference between before treatment and before treatment. The number of CD4 T/CD8 T cells and the number of CD4 T/CD8 T cells were significantly higher than those before treatment, and there was a significant difference between them before treatment and before treatment (P < 0.05). The results of correlation analysis showed that the negative correlation between CD4 T cells and hs-CRP CA-125 was -3.326- 2.910P, respectively, while there was no correlation between CD4 T cells and Cystatin Ccr-GFR (r = 0.2850.101 ~ 0.225P). The negative correlation between 0.05).GFR and Cystatin CCA-125hs-CRP was -2.352- 4.022- 3.022p, respectively. Conclusion before HAART treatment, the serum Cystatin Con CA-125 level in HIVAN patients was significantly increased, and the serum CA-125 level decreased significantly after treatment, and monitoring the serum Cystatin CnCA-125 level was helpful to observe and evaluate the occurrence, progression and prognosis of renal damage in HIVAN patients.
