泌尿系结核的诊疗现状随访结果与分析

【摘要】：[目的]了解泌尿系结核的诊断和治疗现状,并对各种手术方式疗效、服药方法疗效进行简单的探讨。[方法]选取2014年～2016年间昆明医科大学第二附属医院收治的82例泌尿系结核结核患者。对其发病情况、临床症状、影像学资料、实验室资料进行回顾性研究,对手术置管、服药效果进行了评估。[结果]发病高峰年龄段为20～40岁,乡村患者占86. 59%,未接种卡介苗患者占90. 24%。农民及无业者占65. 85%。临床症状依次为尿频56 (68.29%)例、尿急41 (50.00%)例、尿痛40 (48.78%)例、腰痛35 (42.68%)例、血尿24 (29. 27%)、发热 23 (28. 04%)例、盗汗 20 (25. 00%)。肺结核史 24 例(29. 27%)。CT检查80例,明确诊断73例,提示异常80例。γ干扰素释放试验34例,32例明确诊断。76例有手术干预,余药物治疗。置管后行肾切除术9例,未置管肾切除术39例,P0. 05;[结论]肾结核好发年龄为20岁≤年龄≤40岁,且与经济条件、生存环境差、医疗卫生资源缺乏有关。临床症状尿频、尿急、尿痛最为常见。肺结核史或其他部位结核仍然可以为泌尿系结核的诊断提供重要线索。B超、KUB+IVP仍然在泌尿系结核的初筛、诊断中有意义,而CT明显优于上述两种检查。γ干扰素释放试验临床运用前景可观。药物治疗在泌尿系结核治疗中必不可少,而本地区用药相对不规则。输尿管支架置入术可以降低肾切除率。患侧肾脏切除术,仍然是晚期肾结核的主要治疗手段,首选后腹腔镜下肾切除术。
