T-SPOT.TB联合ADA和TB-DNA在诊断结核性胸膜炎中的应用价值

【摘要】:目的探讨结核感染T细胞斑点试验(T-SPOT.TB)技术联合胸水腺苷脱氨酶(ADA)和痰液结核杆菌(TB-DNA)测定对结核性胸膜炎的定性诊断,以达到早期诊断的目的。方法选取唐都医院2014年7月-2015年12月收治的86例结核性胸膜炎患者作为试验组,80例非结核性胸膜炎患者作为对照组,均行T-SPOT.TB检测,同时联合检查患者胸水ADA和痰液TB-DNA,比较T-SPOT.TB、ADA和TB-DNA检测与三者联合检测的应用价值。结果 86例结核性胸膜炎患者,T-SPOT.TB阳性率为84.88%(73/86),ADA阳性率为53.48%(46/86),痰液TB-DNA阳性率为31.40%(27/86),三者联合检测阳性率为93.02%(80/86)。结论 T-SPOT.TB检测联合ADA和痰液TB-DNA检测对结核性胸膜炎的诊断有较高的敏感性,对疑似结核性胸膜炎患者的快速、准确诊断具有重要的辅助诊断价值。
