犬四肢长骨骨折案例分析及最佳治疗方案探讨

【摘要】：目的：为了探讨治疗犬四肢长骨骨折的最佳治疗方案,本研究共统计了187例犬四肢长骨骨折的临床案例,并对其进行了深入分析,以期对临床病例的治疗提供参考。方法：搜集本人工作的宠物医院于2012年1月1日至今的所有四肢骨折相关的临床病例,按发病原因、发病部分、治疗方法和康复情况进行分类统计,并对疗效和个别案例的失败原因进行了分析,根据临床病例的疗效和临床经验制定出最佳治疗方案。结果：1.一共搜集了187个病例。采用的治疗方法有接骨板,髓内针,矫形钢丝,螺钉,其中有单纯使用的,也有配合使用的。前肢发生骨折比例占25.70%,后肢发生骨折占65.78%,多发性占8.56%；前肢肱骨8.59%,桡尺骨17.11%,后肢股骨47.06%,胫腓骨18.72%,在所有骨折病例中,后肢多发。2.所有接诊病例中,大部分预后良好,有几例手术失败。失败的原因有：接骨板错误的放置；螺钉过多导致应力过于集中；钢板选择过短；螺丝钉选择过短；过早负重或功能锻炼；钢板类型选择错误；钢板与螺丝钉不配套。3.制定出最佳治疗方案：首先,明确诊断,全方位X线片的拍摄必不可少；其次,要了解不同植入物的优缺点,能够理解所有植入物的基本信息；第三,手术方案的制定与实施,按照骨科手术的基本原则进行；最后,科学系统的术后护理必不可少。结论：最佳治疗方案的选择和良好的术后护理是治愈犬长骨骨折的基础,本研究根据理论和实践经验而分析总结的最佳治疗方案将对临床犬长骨骨折的治疗起到一定的作用。
[Abstract]:Objective: in order to explore the best treatment scheme for long bone fracture of dog limbs, 187 cases of long bone fracture in dog limbs were counted and analyzed. Methods: to collect all the clinical cases related to the fracture of limbs in the pet hospital from January 1st 2012 to present. The methods of treatment and rehabilitation were classified, and the curative effect and the causes of failure in individual cases were analyzed. According to the curative effect and clinical experience of clinical cases, the best treatment scheme was worked out. Results: 1.A total of 187 cases were collected. The treatment methods used were plate, intramedullary needle, orthopedic wire, screw, among them, only used. The proportion of fractures in the forelimb is 25.70, the fracture in the hind limb is 65.78, and the multiple is 8.56. the humerus of the forelimb is 8.59, the radial ulna 17.11, the femur of the hind limb 47.06 and the tibia and fibula 18.72. In all cases of fracture, the hind limb has multiple fractures. Most of the patients had a good prognosis. The reasons for the failure were as follows: wrong placement of the plate, too much screw leading to stress concentration, too short selection of steel plate, too short choice of screw, premature weight bearing or functional exercise; Wrong choice of plate type; plate and screw do not match .3. to develop the best treatment: first, to make a clear diagnosis, a full range of X-ray photography is essential; second, to understand the advantages and disadvantages of different implants, Ability to understand the basic information of all implants; third, the formulation and implementation of surgery programs, in accordance with the basic principles of orthopedic surgery; finally, Conclusion: the choice of the best treatment plan and the good postoperative nursing are the basis of curing canine long bone fracture. According to the theoretical and practical experience, the best treatment scheme will play an important role in the treatment of canine long bone fracture.
