多囊卵巢综合征患者卵泡发育不良与SCF、BMP-15以及GDF-9表达水平的相关性研究

【摘要】:目的:探讨多囊卵巢综合征(PCOS)患者卵泡中干细胞因子(SCF)、骨形态发生蛋白-15(BMP-15)和生长分化因子-9(GDF-9)与卵泡发育异常的相关性。方法:分别收集经体外受精(IVF)治疗的30例PCOS患者(PCOS组)和40例对照患者(对照组)的卵泡液和颗粒细胞。分析两组患者一般指标情况(包括获卵数,年龄,血清卵泡刺激素,黄体生成素,雌二醇,泌乳素,睾酮以及Gn量)以及卵母细胞MII率,受精率和优质胚胎率并进行比较。采用酶联免疫吸附实验检测卵泡液中SCF,BMP-15和GDF-9的表达水平,应用免疫荧光染色法测定颗粒细胞中SCF的表达水平,并进一步通过蛋白质印迹法比较两组SCF蛋白表达水平。结果:(1)与对照组(n=40)相比,PCOS组(n=30)血清LH,T水平显著增高(P0.05)。其余指标均在统计学上无意义(P0.05);(2)对照组和PCOS组卵母细胞MII率分别为84.67±13.95%和71.10±12.53%,受精率分别为74.37±9.33%和63.26±6.07%,优质胚胎率分别为35.11±3.04%和33.85±2.59%。由此可知,PCOS组卵母细胞MII率和受精率较对照组明显降低(P0.05),然而,两组之间优质胚胎率无明显差异(P0.05);(3)对照组和PCOS组卵泡液中SCF的平均浓度分别为23.05±2.86ng/L和21.17±3.11ng/L,PCOS患者卵泡液中SCF的浓度明显低于正常对照组,差异有统计学意义(P0.05)。免疫荧光检测发现,与对照组相比,PCOS患者颗粒细胞中SCF的表达水平显著下降,随后的蛋白质印迹法实验进一步验证这一结果;(4)卵泡液中对照组和PCOS组BMP-15平均浓度分别为132.53±32.51ng/L和115.50±27.61ng/L。而GDF-9平均浓度分别为138.28±27.81ng/L和121.97±19.66ng/L,可以看出卵泡液中BMP-15和GDF-9的浓度均明显低于正常对照组(P0.05)。结论:(1)PCOS患者卵泡发育不良,卵子质量差,但优质胚胎率与对照组相比无明显差异,提示两组胚胎的发育潜能可能无差异;(2)PCOS组卵泡液和颗粒细胞中SCF的表达均明显降低,提示SCF可能与PCOS卵泡发育异常有关;(3)PCOS患者BMP-15和GDF-9表达水平下调,提示该因子亦可能参与调控PCOS患者卵泡异常发育的过程。
[Abstract]:Objective: to investigate the relationship between (SCF), bone morphogenetic protein-15 (BMP-15) and growth differentiation factor-9 (GDF-9) and follicular dysplasia in patients with polycystic ovary syndrome (PCOS). Methods: follicular fluid and granulosa cells were collected from 30 PCOS patients (PCOS group) and 40 control patients (control group) treated with in vitro fertilization (IVF). The general indexes (including egg count, age, serum follicle stimulating hormone, luteinizing hormone, estradiol, prolactin, testosterone and Gn) and oocyte MII rate, fertilization rate and high quality embryo rate were analyzed and compared between the two groups. Enzyme linked immunosorbent assay (Elisa) was used to detect the expression of SCF- BMP-15 and GDF-9 in follicular fluid. The expression of SCF in granulosa cells was detected by immunofluorescence staining. Furthermore, the expression levels of SCF protein in the two groups were compared by Western blotting. Results: (1) compared with the control group (nm40), the serum LHN T level in the PCOS group was significantly higher than that in the control group (P0.05). The MII rates of oocytes in the control group and PCOS group were 84.67 卤13.95% and 71.10 卤12.53, respectively. The fertilization rates were 74.37 卤9.33% and 63.26 卤6.07%, respectively. The high quality embryo rates were 35.11 卤3.04% and 33.85 卤2.59%, respectively. Therefore, the oocyte MII rate and fertilization rate in PCOS group were significantly lower than those in the control group (P0.05), however, There was no significant difference in the rate of high quality embryos between the two groups (P0.05); (3). The average concentration of SCF in follicular fluid of control group and PCOS group was 23.05 卤2.86ng/L and 21.17 卤3.11ng / L respectively, and the concentration of SCF in follicular fluid of PCOS group was significantly lower than that of normal control group (P0.05). Immunofluorescence assay showed that the expression of SCF in granulosa cells of PCOS patients was significantly decreased compared with the control group, which was further verified by Western blotting assay. (4) the average BMP-15 concentration in follicular fluid was 132.53 卤32.51ng/L and 115.50 卤27.61 ng / L in the control group and PCOS group, respectively. The average concentration of GDF-9 was 138.28 卤27.81ng/L and 121.97 卤19.66ng / L, respectively, which showed that the concentrations of BMP-15 and GDF-9 in follicular fluid were significantly lower than those in normal control group (P0.05). Conclusion: (1) follicular dysplasia and poor egg quality in PCOS patients, but there is no significant difference in the rate of high quality embryos between the two groups, suggesting that there may be no difference in the developmental potential between the two groups, and (2) the expression of SCF in follicular fluid and granulosa cells in the PCOS group is significantly lower than that in the control group. The results suggest that SCF may be related to the abnormal development of PCOS follicles, (3) the expression of BMP-15 and GDF-9 in PCOS patients is down-regulated, which suggests that this factor may also be involved in the regulation of abnormal follicle development in PCOS patients.
