妊娠期糖尿病患者晚孕期血脂水平与新生儿出生体重的相关性研究

【摘要】：目的对妊娠期糖尿病(GDM)患者晚孕期血脂水平变化与新生儿出生体重的相关性进行研究。方法以2013年3月至2013年4月在湖北省天门市第一人民医院就诊的120例行孕期检查的患者为研究对象,其中有43例在孕检过程中发现糖尿病,作为观察组,剩余77例为正常妊娠妇女,作为对照组,对比分析两组患者孕晚期血脂水平与新生儿体重的相关性。结果观察组TG、LDL-C、apoB水平明显高于对照组,差异有统计学意义(P0.05);观察组LDL-C/HDL-C比值明显高于对照组,apoA1/apoB比值明显低于对照组,差异有统计学意义(P0.05);GDM孕妇新生儿出生体重相比高于正常妊娠孕妇,差异有统计学意义(P0.05);新生儿出生体重与孕前BMI、孕周及孕晚期TG呈明显正相关(P0.05),而与孕晚期脂蛋白水平呈明显负相关(P0.05);血脂异常的GDM患者产后出血、早产儿、新生儿窒息、巨大儿和胎儿窘迫的发生率高于血脂正常的GDM患者,差异有统计学意义(P0.05或P0.01)。结论妊娠晚期孕妇均存在着不同程度的高脂血症,新生儿出生体重与孕晚期TG水平呈明显正相关,GDM孕妇脂代谢紊乱较严重,因此加强GDM孕妇孕期饮食指导及血脂监测有重要的临床意义。
[Abstract]:Objective To study the relationship between the changes of blood lipid levels in late pregnancy and the birth weight of newborns in patients with gestational diabetes mellitus (GDM). Results The levels of TG, LDL-C and apoB in the observation group were significantly higher than those in the control group (P 0.05). The LDL-C/HDL-C ratio in the observation group was significantly higher than that in the control group, and the apoA1/apoB ratio was significantly lower than that in the control group. The birth weight of GDM pregnant women was significantly higher than that of normal pregnant women (P 0.05); the birth weight of GDM pregnant women was positively correlated with BMI before pregnancy, TG during pregnancy and late pregnancy (P 0.05), but negatively correlated with lipoprotein level in late pregnancy (P 0.05); and the birth weight of GDM patients with dyslipidemia was positively correlated with BMI before pregnancy, TG during pregnancy and late pregnancy (P 0.05). The incidence of hemorrhage, premature infant, neonatal asphyxia, macrosomia and fetal distress was higher than that of GDM patients with normal blood lipid (P 0.05 or P 0.01). Therefore, it is of great clinical significance to strengthen the dietary guidance and blood lipid monitoring of GDM pregnant women during pregnancy.
