TSP-1、TSP-2、MVD与复发性流产的相关性研究

【摘要】:目的:通过研究复发性流产患者的绒毛、蜕膜组织中血小板反应蛋白-1(TSP-1)、血小板反应蛋白-2(TSP-2)表达及微血管密度(MVD)的大小情况,探讨TSP-1、TSP-2与RSA的关系,为其病因学研究及治疗提供新思路。方法:选择2013年9月-2014年7在山西大医院妇产科门诊因复发性流产行人工流产的患者50例作为研究组,同时选择同期既往有正常足月分娩史,而无不良生育史,本次正常妊娠并自愿行人工流产患者30例作为对照组,在人工流产的同时留取绒毛、蜕膜组织。采用免疫组化SP法检测TSP-1、TSP-2在两组绒毛及蜕膜组织中的表达水平(灰度值越低,表达水平越高)。选用CD34标记血管内皮细胞检测两组绒毛及蜕膜组织中微血管密度(MVD)值。研究结果用SPSS17.0统计软件进行分析,以α=0.05为检验水准。结果:1.绒毛组织中,研究组中TSP-1的的表达明显高于对照组(TSP-1的灰度值显著低于对照组)(P0.05),TSP-2的表达低于对照组(P0.05),TSP-1/TSP-2的比值(表达)高于较对照组(P0.05),差异有统计学意义。在蜕膜组织中,研究组内的TSP-1的表达明显高于对照组(P0.05),TSP-2的表达显著低于对照组,TSP-1/TSP-2比值(表达)高于对照组(P0.05),差异有统计学意义。研究组中,绒毛组织内TSP-1、TSP-2的表达明显高于蜕膜组织(P0.05),TSP-1/TSP-2的(表达)比值高于蜕膜组织(P0.05),差异有统计学意义。对照组中,绒毛组织内TSP-1、TSP-2的表达高于蜕膜组织(P0.05),TSP-1/TSP-2的(表达)比值低于蜕膜组织,差异有统计学意义。2.不论在绒毛组织中还是蜕膜组织中,研究组中MVD值均显著高于对照组,但在绒毛组织中差异有统计学意义,而蜕膜组织中差异无统计学意义。在研究组中,绒毛组织中MVD值显著低于蜕膜组织,差异有统计学意义。在对照组中,绒毛组织中MVD值高于蜕膜组织,差异有统计学意义。3.在绒毛组织中,TSP-1与TSP-2的表达呈正相关,TSP-1,TSP-2的灰度值与MVD值均呈正相关(r分别为0.598、0.556,P0.05),即TSP-1、TSP-2的表达与MVD值呈负相关。在蜕膜组织中,TSP-1与TSP-2的灰度值呈负相关(r=-0.229,P0.05),即二者的表达呈负相关;TSP-1的灰度值与CD34的表达MVD值呈正相关(r=0.360,P0.05),即TSP-1的表达与MVD值呈负相关,而TSP-2的表达与MVD值二者相关性不明显(r=0.017,P0.05)。结论:1.复发性流产患者的绒毛及蜕膜组织中,TSP-1呈高表达状态,TSP-2呈低表达,TSP-1/TSP-2呈高表达,推测TSP-1、TSP-2的表达异常可能是导致复发性流产发生的原因。2.研究组绒毛组织中MVD值明显低于对照组,低于本组蜕膜组织中MVD值,而在正常对照组中,绒毛组织MVD值显著高于蜕膜组织,提示绒毛组织中血管生成障碍是导致部分流产发生的主要原因。3.研究组绒毛组织中TSP-1的表达高于对照组,TSP-2的表达低于对照组,TSP-1/TSP-2的表达显著高于对照组,而MVD值显著低于对照组,推测局部TSP-1过度表达对血管生成障碍的影响可能更大。
[Abstract]:Objective: to investigate the expression of platelet-reactive protein-1 (TSP-1), platelet-reactive protein-2 (TSP-2) and microvessel density (MVD) in villi and decidua of patients with recurrent abortion. The relationship between TSP-2 and RSA provides a new idea for etiology research and treatment. Methods: from September 2013 to July 2014, 50 patients with recurrent abortion in Department of Gynecology and Obstetrics Clinic of Shanxi Great Hospital were selected as study group. 30 cases of normal pregnancy and spontaneous abortion were used as control group, villi and decidua tissue were retained simultaneously. The expression of TSP-1,TSP-2 in chorionic villi and decidua tissues was detected by immunohistochemical SP method (the lower the gray level, the higher the expression level). CD34 labeled vascular endothelial cells were used to detect the microvessel density (MVD) in chorionic villi and decidua. The results were analyzed by SPSS17.0 software, and the test level was 伪 = 0. 05. The result is 1: 1. In chorionic villi, the expression of TSP-1 in the study group was significantly higher than that in the control group (P 0.05), and the expression of TSP-2 in the study group was significantly lower than that in the control group (P0.05). The ratio of TSP-1/TSP-2 (expression) was higher than that of control group (P0.05), the difference was statistically significant. In decidual tissues, the expression of TSP-1 and TSP-2 in the study group was significantly higher than that in the control group (P0.05), and the TSP-1/TSP-2 ratio was significantly higher than that in the control group (P0.05). The difference is statistically significant. In the study group, the expression of TSP-1,TSP-2 in chorionic villi was significantly higher than that in decidua (P0.05), and the ratio of TSP-1/TSP-2 was higher than that in decidua (P0.05). In the control group, the expression of TSP-1,TSP-2 in chorionic villi was higher than that in decidual tissue (P0.05), and the ratio of TSP-1/TSP-2 (expression) was lower than that in decidua tissue, the difference was statistically significant. Both in chorionic villi and decidua tissues, MVD values in study group were significantly higher than those in control group, but there was significant difference in chorionic villus tissue, but no significant difference in decidua tissue. In the study group, MVD in chorionic villi was significantly lower than that in decidua tissue, and the difference was statistically significant. In the control group, the MVD value in chorionic villi was higher than that in decidua tissue, and the difference was statistically significant. In chorionic villi, the expression of TSP-1 and TSP-2 was positively correlated, and the gray value of TSP-1,TSP-2 was positively correlated with the value of MVD (r = 0.5980.556p0.05), that is, the expression of TSP-1,TSP-2 was negatively correlated with the value of MVD. In decidua tissue, the gray value of TSP-1 and TSP-2 was negatively correlated (r = -0.229, P 0.05), that is, the expression of TSP-1 and TSP-2 was negatively correlated. There was a positive correlation between the gray value of TSP-1 and the expression of MVD of CD34 (r = 0.360 0 P 0.05), that is, the expression of TSP-1 was negatively correlated with the value of MVD, but the expression of TSP-2 was not correlated with the value of MVD (r 0.017 P 0.05). Conclusion: 1. In the villus and decidua of recurrent abortion, the expression of TSP-1 was high, the expression of TSP-2 was low, and the expression of TSP-1/TSP-2 was high. We speculated that TSP-1, was expressed in patients with recurrent abortion. Abnormal expression of TSP-2 may be the cause of recurrent abortion. 2. 2. The MVD value of chorionic villi in the study group was significantly lower than that in the control group and the MVD value in decidua tissue, while in the normal control group, the MVD value of chorionic villi was significantly higher than that of decidua tissue. It is suggested that the main cause of partial abortion is vascular dysfunction in chorionic villi. 3. 3. The expression of TSP-1, TSP-2, TSP-1/TSP-2 and MVD in the chorionic villi of the study group were significantly higher than those in the control group. It is speculated that local overexpression of TSP-1 may have a greater effect on angiogenesis disorder.
