宫颈机能不全非孕期不同术式干预后的临床比较

【摘要】：目的:比较宫颈机能不全患者非孕期行腹腔镜下子宫峡部环扎术与经阴道子宫峡部环扎术的治疗效果。方法:回顾性分析2013年5月至2015年5月于我院确诊为宫颈机能不全的非孕患者,接受腹腔镜或经阴道子宫峡部环扎术后5月内自然受孕,单胎并随访资料完整的病例共计63例,其中接受腹腔镜下子宫峡部环扎术患者30例(研究组),经阴道子宫峡部环扎术患者33例(对照组),对两种手术方式的临床效果进行比较。结果:研究组治疗后孕周≥28周的成功率(96.67%)显著高于对照组(51.52%)(P0.05),研究组术后平均分娩孕周较对照组延长(P0.05),研究组的流产率(3.33%)、早产率(10.00%)较对照组低(48.48%,30.30%),足月产率(86.67%)较对照组高(21.21%),差异有统计学意义(P0.05)。研究组患者手术时间(37.27±1.93分钟)及住院时间(5.17±0.38天)短于对照组(52.30±2.36分钟,8.24±2.22天),术中出血量(13.13±1.57 ml)少于对照组(31.61±1.87 ml),差异有统计学意义(P0.05)。研究组与对照组术后并发症发生率分别为0、18.18%,差异有统计学意义(P0.05)。结论:非孕期腹腔镜下子宫峡部环扎术治疗宫颈机能不全相比经阴道子宫峡部环扎术临床治疗效果更好,具有较高的安全性,值得临床推广。
[Abstract]:Objective: to compare the effect of laparoscopic uterine isthmus ligation and transvaginal uterine isthmus ligation in non-pregnant patients with cervical insufficiency. Methods: from May 2013 to May 2015, 63 cases of non-pregnant patients diagnosed as cervical insufficiency in our hospital were treated with laparoscopy or transvaginal uterine isthmus ligation within 5 months, and 63 cases with single pregnancy and complete follow-up data were analyzed retrospectively.
