【摘要】：背景和目的:脑脊液分流术常被用于治疗顽固性特发性颅内压增高症,但对其疗效进行评价的研究十分缺乏。本研究是目前已知国内仅有的、规模最大的对现有脑室-腹腔分流术或腰大池-腹腔分流术的相关文献进行的荟萃分析。材料与方法:检索1988至2014年所有同行评议杂志上发表的脑脊液分流术治疗特发性颅内压增高症的文献,提取数据绘制表格,试图通过对定性资的率进行χ2检验揭示脑脊液分流术治疗特发性颅内压增高症的普遍规律。结果:纳入435例,术后54%视力改善、80%头痛缓解、70%视乳头水肿消除,43%需再次手术。大小并发症发生率分别为7.6%和32.9%。头痛、视乳头水肿和视力下降的缓解率,分流失败和术后低颅压的发生率的χ2检验有显著性意义,而再次手术率和分流管阻塞的发生率等的χ2检验则为阴性结果。结论:尽管存在着巨大的局限性,本研究初步揭示了脑脊液分流术不同术式治疗特发性颅内压增高症疗效和并发症的异同,而有关观点的确证则有待于随机对照试验或有对照的观察性研究的实行。
[Abstract]:Background and objective: cerebrospinal fluid shunt (CSF shunt) is often used to treat intractable idiopathic intracranial hypertension, but there is very little research to evaluate its curative effect. Largest meta-analysis of available literature on ventriculoperitoneal shunt or lumbar cisternoceliac shunt. Materials and methods:. To search the literature published in all peer-reviewed journals from 1988 to 2014 on the treatment of idiopathic intracranial hypertension with cerebrospinal fluid shunt. The data were drawn out and the 蠂 2 test was used to reveal the general rule of cerebrospinal fluid shunt in the treatment of idiopathic intracranial hypertension. Results: 54% cases of postoperative visual acuity were improved. 80% 70% of headache relief and 43% of the elimination of papillary edema required reoperation. The incidence of complications was 7.6% and 32.9.The remission rate of headache, papillary edema and visual acuity decreased. The 蠂 2 test of shunt failure and postoperative low intracranial pressure was significant, but the rate of reoperation and the incidence of shunt obstruction were negative. Conclusion: although there are great limitations. This study preliminarily revealed the similarities and differences of the effects and complications of cerebrospinal fluid shunt in the treatment of idiopathic intracranial hypertension. The corroboration of the views is to be carried out in randomized controlled trials or controlled observational studies.
