葡萄球菌感染的膝关节置换兔局部低强度超声波刺激观察

【摘要】:目的观察葡萄球菌感染的膝关节置换兔局部低强度超声波刺激的效果。方法清洁级SD兔40只,采用膝关节试件(万古霉素骨水泥制作)对兔膝关节行膝关节置换,术后随机分为观察组和对照组各20只。术后第3天取两组兔于膝关节处紧贴膝关节后方穿刺注射金黄色葡萄球菌菌液0.1 m L1次后,观察组予低强度超声辐射,1次/d,连续5 d。对照组不做任何处理。术后第7天,采集两组颈动脉血检测两组血清万古霉素。术后第30天观察两组膝关节炎症情况,术后第30天处死兔,观察膝关节液金黄色葡萄球菌培养情况,观察膝关节滑膜组织感染情况。结果术后第7天观察组、对照组血清万古霉素水平分别为(60.23±8.36)、(40.36±6.28)mmol/L,两组比较,P0.05。术后第30天时观察组、对照组炎症评分分别为(2.13±0.08)、(3.12±1.02)分,两组比较,P0.05。观察组对照组膝关节附近、关节腔内均可见炎症,但观察组炎症范围信号低,范围小。观察组金黄色葡萄球菌生长较少,甚至没有菌落生长,对照组可见大量淡黄色的阳性葡萄球菌菌落。术后第30天时观察组、对照组感染评分分别为(2.23±0.79)、(4.84±0.13)分,两组比较,P0.05。结论局部低强度超声波刺激可防治膝关节置换术后早期感染。
[Abstract]:Objective to observe the effect of local low-intensity ultrasonic stimulation in rabbits with staphylococcal infection. Methods knee joint replacement was performed in 40 SD rabbits with clean grade by using vancomycin bone cement. The rabbits were randomly divided into two groups: the observation group and the control group. On the third day after operation, the rabbits in both groups were injected with 0.1 m L1 staphylococcus aureus at the back of the knee joint. The observation group was treated with low intensity ultrasound irradiation for 5 days. No treatment was given in the control group. On the 7th day after operation, two groups of carotid blood samples were collected to detect vancomycin. The knee joint inflammation was observed on the 30th day after operation and the rabbits were killed on the 30th day after operation. The culture of staphylococcus aureus in the knee joint fluid was observed and the infection of synovial tissue of the knee joint was observed. Results on the 7th day after operation, the serum vancomycin levels in the observation group and the control group were 60.23 卤8.36 卤6.28 mmol / L, 40.36 卤6.28 渭 mol / L, respectively, compared with the control group (P 0.05). On the 30th day after operation, the inflammatory scores of the control group were 2.13 卤0.08 and 3.12 卤1.02, respectively, compared with that of the control group (P 0.05). Inflammation was seen in the joint cavity near the knee joint in the observation group, but the range of inflammation was low and small in the observation group. The growth of Staphylococcus aureus in the observation group was less than that in the control group, and a large number of yellowish positive staphylococcus colonies were found in the control group. On the 30th day after operation, the infection score of the control group was 2.23 卤0.79 and 4.84 卤0.13, respectively, compared with that of the control group (P 0.05). Conclusion Local low-intensity ultrasound stimulation can prevent early infection after knee arthroplasty.
