乳腺癌根治术后早期化疗对患者临床预后的影响

【摘要】：目的:探讨乳腺癌根治术后不同时间开始化疗对患者临床预后的影响。方法:自2012年1月—2013年1月,前瞻性收集我院收治的乳腺癌患者100例,根据患者术后化疗时间,将患者分为观察组(开始时间3周)和对照组(开始时间3~8周)。主要观察指标包括临床疗效(无进展生存期、生存率和复发率)和安全性(治疗相关并发症和健康相关的生存质量)。结果:与对照组相比,观察组患者无进展生存期明显延长(P=0.049);3年复发率明显降低(20.00%vs.38.00%,P=0.047)。两组患者生存率无统计学差异(94.00%vs.92.00%,P=1.000)。观察组患者术后1年健康相关的生存质量显著高于对照组[(73.00±12.16)vs.(64.64±10.58),t=3.677,P=0.000]。两组患者主要治疗相关并发症包括骨髓抑制、贫血、腹泻、恶心和呕吐等差异均无统计学意义(P0.05)。结论:术后早期化疗有助于延长乳腺癌患者无进展生存期,改善患者术后生存质量。
[Abstract]:Objective: to investigate the effect of chemotherapy on the clinical prognosis of patients with breast cancer at different time after radical mastectomy. Methods: from January 2012 to January 2013, 100 patients with breast cancer admitted in our hospital were prospectively collected. According to the time of postoperative chemotherapy, the patients were divided into observation group (starting time 3 weeks) and control group (starting time 3 weeks and 8 weeks). The main outcome measures included clinical efficacy (progression-free survival, survival rate and recurrence rate) and safety (treatment-related complications and health-related quality of life). Results: compared with the control group, the progression-free survival time of the patients in the observation group was significantly longer than that of the control group, and the recurrence rate of 3 years was significantly decreased by 20.00 vs.38.00. There was no statistical difference in survival rate between the two groups. The health-related quality of life in the observation group was significantly higher than that in the control group one year after operation [73.00 卤10.58 12.16)vs.(64.64 卤3.677 P0. 000]. There was no significant difference in major treatment-related complications between the two groups, including bone marrow suppression, anemia, diarrhea, nausea and vomiting. Conclusion: early postoperative chemotherapy can prolong the progressive survival of breast cancer patients and improve the postoperative quality of life.
