退变性腰椎滑脱中椎小关节退变分级及椎旁肌MRI信号改变与下腰痛相关性研究

【摘要】：目的研究退变性腰椎滑脱中椎小关节退变分级及椎旁肌MRI信号改变与下腰痛相关性,为早期下腰痛做出初步的病因诊断及为下一步的治疗方案提供指导意义。方法选取宁夏医科大学总医院脊柱骨科2014年12月-2016年12月期间住院的腰椎退变性滑脱患者80例为临床资料,分别统计出患者年龄、性别及椎小关节退变程度,用统计学分析二者的相关性,其次研究椎小关节退变分级与下腰痛之间的关系;通过腰椎核磁共振成像来测量下腰痛患者的椎旁肌横截面积、椎间盘面积,并计算出二者的比值,研究其与下腰痛之间的关系。结果患者的性别和椎小关节退变分级之间没有关系(t=0.387,P=0.680);椎小关节退变分级与年龄有关系(r=0.664,P0.001);80例患者的腰椎小关节存在不同等级的退变,其中,3级20例,2级31例,1级29例;腰痛患者疼痛评分(视觉模拟评分)结果显示:29例1级患者中,24例分值3分,5例在4~6分;31例2级患者中,26例分值在4~6分,5例3分;20例3级患者中,16例分值在7~10分,4例在4~6分。表明患者的关节突关节退变分级与下腰痛之间具有明确联系(r=0.879,P0.001);患者椎旁肌的横截面积与椎间盘面积比值的大小与下腰痛之间具有一定的关系。结论退变性腰椎滑脱中椎小关节退变分级及椎旁肌MRI信号改变与下腰痛之间存在相关性。
[Abstract]:Objective to study the correlation between the grade of degenerative facet joint degeneration and MRI signal of paraspinal muscle in degenerative lumbar spondylolisthesis. Methods 80 cases of lumbar degenerative spondylolisthesis hospitalized in Department of Spinal Orthopedics, General Hospital of Ningxia Medical University from December 2014 to December 2016 were selected as clinical data. The relationship between lumbar facet joint degeneration and low back pain was studied by statistical analysis, and the cross sectional area of paravertebral muscle and intervertebral disc area were measured by lumbar magnetic resonance imaging. The ratio of the two was calculated and the relationship between them and low back pain was studied. Results there was no correlation between the sex of the patients and the grade of facet joint degeneration. There was no correlation between the sex of the patients and the grade of degenerative vertebral facet joints. There was no correlation between the grade of degeneration of the facet joints and the age of the patients, and there were different grades of degeneration of the lumbar facet joints in 80 patients, including 20 cases of grade 3 and 31 cases of grade 1 and grade 2. The results of pain score (visual analogue score) of 29 patients with low back pain showed that among 29 patients with grade 1, 24 patients had a score of 3 scores, 5 patients with grade 2 patients with grade 4 to 6 patients, 26 patients with a score of 26 patients with a score of 4 to 6 patients with a score of 5 patients with grade 3 and 20 patients with grade 3 patients. The scores of 16 patients were between 710 and 10 and 4 patients with grade 2 patients had a score of 4 to 6 points. The results showed that there was a clear relationship between the degenerative grade of articular facet and lower back pain, and there was a certain relationship between the cross sectional area of paravertebral muscle and the ratio of intervertebral disc area and lower back pain. Conclusion in degenerative lumbar spondylolisthesis there is a correlation between degenerative grade of facet joint and MRI signal of paravertebral muscle and lower back pain.
