新疆塔城地区哈萨克族胫骨平台几何参数测定的临床意义

【摘要】:[目的]通过对新疆哈萨克族和汉族胫骨平台几何参数的比较,建立起新疆哈萨克族胫骨平台特征性几何参数数据库,掌握汉族与哈萨克族民族间解剖上所存在的差异性,为哈萨克族手术技巧及手术器械的个体化设计及选择提供数据资料。[方法]在本院和合作医院分别随机抽取病房、门诊患者及健康志愿者汉族100例(200膝,其中男100膝,女100膝)、哈萨克族100例(200膝,其中男100膝,女100膝),年龄25~50岁,平均35岁。通过CT和三维重建技术完成胫骨平台薄层扫描及3D影像资料。用Volume Rendering软件实现膝关节胫骨平台几何参数的测定,包括测量胫骨平台线性参数与角度参数,并进行统计学分析。[结果]测量结果分别按照性别分组与族别配对分组,胫骨平台各项线性指标男性组均大于女性组,在同一民族之内,差异更加显著(P0.05)。同一性别相比较,哈萨克族的相关线性制表明显大于汉族(P0.05),女性之间的差异尤为显著(P0.01)。胫骨平台角度参数比较,胫骨平台后倾角与内翻角同一族别中男女无明显差异,但同一性别中,哈萨克族明显大于汉族。[结论]哈萨克族的胫骨平台线性参数与角度参数均与国人平均水平有差异;只有充分考虑到哈萨克族解剖的特异性,才能很好的完成手术,预防围手术期并发症。
[Abstract]:[objective] by comparing the geometric parameters of tibial plateau between Kazak and Han nationality in Xinjiang, we established the database of characteristic geometric parameters of tibial plateau of Kazak nationality in Xinjiang, and grasped the anatomical differences between Han nationality and Kazak nationality. To provide data for individual design and selection of Kazak surgical techniques and instruments. [methods] A total of 100 patients (200 knees, 100 male and 100 female) and 100 Kazakh (100 knees, 100 male and 100 female) were randomly selected from our hospital and the cooperative hospital. The average age was 35 years (25 ~ 50 years). Thin layer scanning and 3 D imaging of tibial plateau were performed by CT and 3 D reconstruction. The geometric parameters of tibial plateau of knee joint were measured by Volume Rendering software, including the linear parameter and angle parameter of tibial plateau, and the statistical analysis was carried out. [results] the results showed that the tibial plateau linear index in male group was larger than that in female group, and the difference was more significant in the same ethnic group (P0.05). Compared with the same sex, the correlation linear table of Kazak was significantly larger than that of Han (P0.05), especially the difference between women (P0.01). Comparison of tibial plateau angle parameters showed that there was no significant difference between male and female in tibial plateau posterior inclination angle and varus angle, but in the same sex, Kazak nationality was obviously larger than Han nationality. [conclusion] the linear parameters and angle parameters of tibial plateau of Kazakh people are different from the average level of Chinese, only when the specificity of the anatomy of Kazakh nationality is fully considered, the operation can be completed well and the perioperative complications can be prevented.
