高血压患者动脉硬化与中心动脉压及颈动脉内膜厚度的关系

【摘要】:目的:探讨原发性高血压患者踝臂动脉脉搏波传导速度与中心动脉压及颈动脉内膜中层厚度的相关性。方法:对2010年1月—2014年3月在新疆医科大学第一附属医院高血压科住院治疗的444例原发性高血压患者进行回顾性研究,入选对象年龄(56.63±8.86)岁。对所有患者进行颈动脉内膜中层厚度(IMT),踝臂动脉脉搏波传导速度(baPWV),中心动脉压(CAP)的测定。根据踝臂动脉脉搏波传导速度(baPWV)分为异常组(baPWV≥14m/s, n=208)和正常组(baPWV14m/s, n=236)。用Pearson简单相关分析高血压患者baPWV与CAP及IMT的相关性。采用Logistic回归分析baPWV与CAP及IMT的关系。结果:baPWV异常组的年龄、24hSBP、dSBP、nSBP、CASP、 CADP、CAPP、AP、AIx、AIx75、IMT均要高于正常组,差异有统计学意义(P0.05)。Pearson相关分析显示baPWV与年龄、24hSBP、dSBP、nSBP、CASP、CAPP、 AP、AIx、AIx75、IMT相关(r值分别为0.385,0.384,0.373,0.279,0.609,0.364,0.569,0.394,0.138,0274,0.513,均P0.05),进一步控制年龄、24hSBP、dSBP、nSBP后,CASP、CADP、CAPP、AP、AIx75、IMT仍与baPWV相关。Logistic回归分析显示CASP、CAPP、IMT与baPWV相关(OR值分别为1.065,1.099,29.467,均P0.05)。结论:CASP、CAPP、IMT是原发性高血压患者baPWV增加的危险因素。
[Abstract]:Objective: to investigate the correlation between pulse wave velocity of ankle brachial artery and central arterial pressure and intima-media thickness of carotid artery in patients with essential hypertension. A retrospective study was conducted on 444 patients with essential hypertension who were hospitalized in the Department of Hypertension of the first affiliated Hospital of Xinjiang Medical University from January 2010 to March 2014.
