强化剂量阿托伐他汀对不稳定型心绞痛患者经皮冠脉介入术围手术期B7-H4的影响

【摘要】：目的:探讨强化剂量阿托伐他汀对不稳定型心绞痛患者经皮冠脉介入术(PCI)围手术期B7-H4的表达的影响。方法:选取2012年10月至2014年10月广西医科大学第一附属医院和广西中医药大学第一附属医院心内科收治的不稳定型心绞痛患者80例,随机分为常规剂量阿托伐他汀组和强化剂量阿托伐他汀组,每组40例,两组患者分别于PCI术前及术后18~24 h采集外周静脉血,采用酶联免疫吸附法(ELISA)检测血清可溶性B7-H4(s B7-H4)、白介素(IL)-4、IL-10、γ干扰素(IFN-γ)水平,荧光定量PCR(FQ-PCR)法检测外周血单核细胞(PBMC)B7-H4 m RNA相对表达量。结果:两组患者PCI术后的IL-10、s B7-H4水平和B7-H4 m RNA表达量均较术前升高,其中强化剂量组升高更为显著(P0.05)。两组患者PCI术后IL-4、IFN-γ水平均较术前降低,且强化组较常规组下降更为明显(P0.05)。结论:强化剂量阿托伐他汀可能通过促进B7-H4表达,调节炎性因子IL-4、IFN-γ水平,从而抑制不稳定型心绞痛患者PCI术后免疫炎症。
[Abstract]:Objective: to investigate the effect of intensive dose Atto vastatin on the expression of B7-H4 in patients with unstable angina pectoris undergoing percutaneous coronary intervention (PCI). Methods: from October 2012 to October 2014, the first affiliated Hospital of Guangxi Medical University was selected. And 80 patients with unstable angina pectoris admitted to the Department of Cardiology, the first affiliated Hospital of Guangxi University of traditional Chinese Medicine, They were randomly divided into routine dose Atto vastatin group and enhanced dose Atto vastatin group with 40 patients in each group. Peripheral venous blood was collected before and after PCI for 1824 hours. Elisa was used to detect the levels of serum soluble B7-H _ 4s B _ 7-H _ 4N, IL-10 and IFN- 纬. The relative expression of B7-H4 m RNA in peripheral blood monocytes was detected by fluorescence quantitative PCR- FQ-PCR.Results: the levels of IL-10s B7-H4 and the expression of B7-H4m RNA in peripheral blood monocytes were increased after PCI in both groups. The levels of IL-4 and IFN- 纬 after PCI in both groups were lower than those in the preoperative group, and the levels of IL-4 and IFN- 纬 in the enhanced group were significantly lower than those in the normal group. Conclusion: the enhanced dose of Atto vastatin may promote the expression of B7-H4. To regulate the level of IL-4 and IFN- 纬 in patients with unstable angina pectoris and suppress the immune inflammation after PCI.
