冠状动脉支架置入后心肌脂肪浸润的CT表现

【摘要】:目的研究冠状动脉支架置入后心肌脂肪浸润的双源CT表现。方法回顾性分析行双源CT冠状动脉造影的患者160例,分析冠状动脉支架置入后心肌脂肪浸润的分布特征,并评估部分病例的左心室首过灌注功能。结果 160例患者中,有心肌脂肪浸润者24例;有8例患者多支血管置入了支架;16例患者单支血管置入支架,包括左前降支11例,右冠状动脉1例,回旋支4例。支架置入年限为0.8~15(5.4±3.4)年。心肌脂肪浸润主要位于支架远端心肌,表现为局部心肌强化减低、心室壁变薄,而且心肌脂肪浸润区域出现灌注减低。选取6例心肌脂肪浸润患者和27例无脂肪浸润患者进行左心室首过灌注功能比较,发现支架植入后心肌脂肪浸润患者心搏出量、舒张末期容积明显高于无脂肪浸润者(P均0.05)。结论冠状动脉支架置入后心肌脂肪浸润一般好发于支架远端心肌,同时伴有心脏功能异常。双源CT对冠状动脉支架置入后心脏可同时提供形态学和功能学信息。
[Abstract]:Objective to study the dual-source CT findings of myocardial fat infiltration after coronary stenting. Methods the distribution of myocardial fat infiltration after coronary stenting was analyzed retrospectively in 160 patients who underwent double source CT coronary angiography. Results among 160 patients, 24 had myocardial fat infiltration, 8 patients had multiple vessels and 16 patients had single vessel stents, including 11 patients with left anterior descending branch. Right coronary artery (1 case), circumflex branch (4 cases). Stent placement was 0.8155.4 卤3.4 years. Myocardial fat infiltration was mainly located in the distal myocardium of the stent, which was characterized by decreased local myocardial enhancement and thinning of ventricular wall. The left ventricular first-pass perfusion was compared in 6 patients with myocardial fat infiltration and 27 patients without fat infiltration, and the cardiac output was found in the patients with myocardial fat infiltration after stent implantation. The end diastolic volume was significantly higher than that in the patients without fat infiltration (P < 0.05). Conclusion Myocardial fat infiltration after coronary stent implantation is more common in the distal end of the stent than that in the distal end of the stent. Dual-source CT can provide both morphological and functional information after coronary stent implantation.
